Transition of Care

What does the term 'transition of care' mean and why is it important to me as a healthcare provider? 

Transition of care is defined as 'the movement of patients, residents and clients from one health care setting to another'. Transition of care can occur between facilities (such as from one hospital to another hospital) or it can occur within a facility (e.g., a transfer between units within the same facility such as from the Emergency department to an inpatient unit).

Additionally, think about the transition of care that occurs from one shift to the next (e.g., between the night shift and the day shift). Transfer of care among health care providers has been identified as an occupational health and safety issue for health care workers and a major contributor to employee exposure to hazards and can result in unsafe client care. This is mainly due to the lack of a clear communication plan between healthcare providers. Failure of healthcare providers to communicate critical information about a transitioning client places not only employee safety but also client safety at risk and may lead to negative outcomes for both employees and clients.

Staffing Office Reminders:
  • Lateness - remember that you are required to arrive 15 minutes before the start of your shift so that you have time to organize yourself and listen to the report. If you are running late, please contact the Carecor Staffing Office immediately so that we can inform the facility of your time of arrival. 
  • Late Booking - if you are booked for an urgent assignment where it is known in advance that you will be arriving after the normal start of the assignment, please contact the Carecor Staffing Office immediately upon your arrival at the unit so that we enter the correct start time of your assignment. 
  • Additional Hours Worked Form - ensure you carry this form whenever you are booked for an assignment. You need to have this form signed by an authorized Charge Nurse at the facility so that you are paid for any extra hours that you are assigned to work. This form needs to be submitted before you leave the assignment.
  • Changes to Your Shift - contact the Carecor Staffing Office if your shift changes from the original booking to ensure that we are not overcharging the faciity or underpaying you. For example, if you were originally booked for a 12-hour assignment and they unit only needs you for 8-hours, you must notify Carecor Staffing Office. 
  • Changes to Your Assigned Unit - if the unit changes either upon arrival at your assignment, or at some point during your assignment, please sign out of the unit where you are leaving and sign-in to the new unit. You also need to advise the Carecor Staffing Office immediately so we can change your schedule in our computer scheduling system.
