Hartford Foundation Trustees Approve Two Grants Totaling $2.275 million

The John A. Hartford Foundation Board of Trustees has approved two grants totaling $2.275 million. One supports the implementation of a transformation business plan to expand the availability of palliative care and enhance the sustainability of longtime grantee the Center to Advance Palliative Care (CAPC), while the other aids Grantmakers In Aging’s  goal of growing its membership and increasing the number of foundations engaged in aging-related philanthropy.

Hartford Change AGEnts Updates

The Hartford Change AGEnts Initiative Policy Institute will provide an opportunity for Hartford Change AGEnts to gain new knowledge and acquire skills necessary to mobilize for action on policy issues that bring about improvements in health care and quality of life for older adults. Institute topics will range from federal policy and regulatory change to state and local policy work.

The Institute, to be held June 22-24, 2014 in Washington D.C., will help AGEnts gain a stronger understanding of the complex policy-making environment and the role that they can play in driving policy outcomes.

Congratulations to the five teams selected to receive Collaborative Pilot Grants for innovative interdisciplinary projects designed to effect change in health care practice. The grants were awarded through a joint program between the Hartford Centers of Excellence in Geriatric Medicine and Psychiatry and the Hartford Change AGEnts.


The Hartford Change AGEnts Initiative is pleased to announce the Beeson Scholars/Hartford Change AGEnts Action Fund, which provides a new funding opportunity for current and alumni Beeson Scholars.  The Hartford Foundation and American Federation for Aging Research (AFAR) are seeking applications for projects that align with the new Hartford Change AGEnts Initiative focused on practice change and interprofessional collaboration with other Change AGEnts.  5-10 grants ranging from $5,000 to $20,000 will be available on a rolling basis.

The Hartford Change AGEnts Initiative is creating a document, What Is Practice Change?, that provides a working definition of practice change within health care to guide project activities, training and messaging, and help others understand the work of the Change AGEnts.

Excellent New Resources from Hartford Grantees

Check out the latest resources and information you can use from our Hartford grantees:
  • The Eldercare Workforce Alliance, a coalition of 30 national organizations committed to strengthening the workforce needed to care for older adults, has released state-specific briefs showing that the United States is unprepared to meet the current and future workforce needs for its older adult population.
  • Four specialty societies have released approved guidelines to help ensure that high-quality care is delivered in geriatric emergency departments. For more information, read Marcus Escobedo’s Health AGEnda post: “New Guidelines Show What a Geriatric Emergency Department Should Look Like."
  • And the American Geriatrics Society has released the second list of five tests and treatments that older adults and their healthcare providers should question and discuss.   The recommendations were recently highlighted in Paula Span’s The New Old Age blog,  which included an interview with Beeson scholar Dr. Sei Lee of the University of California, San Francisco, a member of the panel that developed the questions.
