- CHA Client ID number (or the HCV Tenant ID/Voucher number)*
- Last 4 digits of Head of Household's Social Security Number (SSN)*
- Applicant date of birth*
- A valid email address
*These items are required to verify CHA residency. Applications will be reviewed and participants selected based on applicant interest and number of slots available.
