Patient Health Records 

The House of Commons Library briefings:  

Patient health records and confidentiality. This briefing describes how patients may request access to their records, and the circumstances in which access to the records of others may be allowed. It also describes statutory and public interest disclosures of patient information; information sharing rules for people who lack mental capacity; and access to information on hereditary conditions for relatives.
 
-  A paperless NHS: electronic health records.  This briefing provides information regarding the Government’s decision to commit to making all patient and care records digital, real-time and inter-operable by 2020.
Nurse Revalidation 

The Nursing and Midwifery Council (NMC) Revalidation Round-Up, published July 2016.  This update shows latest revalidation figures, including 90.48 per cent of nurses and midwives have successfully renewed their registration.

Revalidation: Nursing Demands Evidence Webinar 
21st July 2016, 3-4pm.  A short webinar, which looks at the Nursing and Midwifery Council (NMC) themes and demonstrates how The Royal Marsden Manual of Clinical Nursing Procedures supports each theme of the code. 
Community Care and Hospital Discharge

District nursing in the Netherlands

The Royal College of Nursing (RCN) has updated The Buurtzorg Nederland (home care provider) model:observations for the United Kingdom
This briefing examines the success of the Buurtzorg community care programme in the Netherlands; the growth of interest in the UK for replicating this success; and the challenges that will need to be addressed to realise this.  The briefing has been updated to reflect developments taking place across the UK with regards to piloting Buurtzorg, as well as the growth of RCN about how the model works.

Delayed discharges and hospital type

The Centre for Health Economics has published Delayed discharges and hospital type: evidence from the English NHS (Research paper 133).  This report investigates how delayed discharges vary by hospital type and the extent to which such differences can be explained by demography, casemix, the availability of long-term care and hospital governance.  It finds that a greater local supply of long-term care (care home beds) is associated with fewer delays. 

Discharging older patients from hospital
The National Audit Office has published Discharging older patients from hospital.  The report finds that the health and social care system’s management of discharging older patients from hospital does not represent value for money.

Unsafe discharge from hospital
The Parliamentary Health Service Ombudsman has published A report of investigations into unsafe discharge from hospital.  This report focuses on the increasing number of complaints that the service is receiving regarding people being discharged from hospital with poor planning, co-ordination and communication between hospital staff and between health and social care services.  
Patient Involvement

Patient activation measure: feasibility and rollout
NHS England, in conjunction with the University of Leicester, has published Independent evaluation of the feasibility of using the Patient Activation Measure in the NHS in England: summary interim report.  It focuses on practical lessons and points to consider for organisations who wish to use the Patient Activation Measure (PAM) including the: need for clarity and a full understanding of why the PAM tool is being used and what function it is serving; whether using PAM purely as an outcome measure is appropriate; value of PAM in starting the conversation about person-centred care; and challenges of integrating PAM into existing and new services.  NHS England has agreed a five-year licence to expand the use of the PAM tool with up to 1.8 million people, as a core component of the developing Self-Care programme.

Developing a patient and public involvement panel for quality improvement - an updated step-by-step guide to developing an effective patient led panel. For patients, the public, and staff working in quality improvement in NHS healthcare provider organisations.

Patients as partners
Published by The King’s Fund: Patients as partners: building collaborative relationships among professionals, patients, carers and communities. This report stems from an evolving body of the Fund's work focused on exploring and supporting shared leadership. This work is reinforced by a growing consensus that health services, agencies, patients and communities need to work together more and differently. 

Review of how well people are involved in their own care
Better Care in My Hands (CQC) looks at how people are being involved in their own care.  It is based on analysis of CQC’s national and thematic reports, such as State of Care, its inspection findings and on its NHS patient surveys. 
Safeguarding

Child safeguarding and health care for looked after children
 
The Care Quality Commission has published Not seen, not heard: a review of the arrangements for child safeguarding and health care for looked after children in England.  This report looks at how effectively health services provide early help to children in need, how they identify and protect children at risk of harm and looked-after children’s health and wellbeing. It reviews findings from 50 inspections across England as well as specific focus groups.
Evidence-Based Policy Making

Published by the Care Quality Commission: Learning from serious incidents in NHS acute hospitals.This briefing document discusses the need for a change in the way that serious incidents are investigated and managed in the NHS.  It reviews a sample of serious incident investigation reports from 24 acute hospital trusts.

Use of e-cigarettes in public places and workplaces: Advice to inform evidence-based policy making

NHS staff health and wellbeing CQUIN
NHS staff health & wellbeing: CQUIN supplementary guidance outlines the need for the NHS to improve its offer as an employer to look after the health and wellbeing of its staff.   It provides additional context about the need for focussing on health and wellbeing; details of the content of the schemes, drawing on best practice examples; information about the methods of assurance and processes; and outlines future support available during 2016/17.

Female genital mutilation
The World Health Organisation guidelines on the management of health complications from female genital mutilation. These guidelines are intended primarily for health-care professionals involved in the care of girls and women who have been subjected to any form of female genital mutilation (FGM). This document also provides guidance for policy-makers, health-care managers and others in charge of planning, developing and implementing national and local health-care protocols and policies.

Role of the nurse on the CCG governing body
NHS Clinical Commissioners Nurses Forum has published a briefing report on the role of the nurse on the CCG governing body.  Through a series of case studies the report shows the impact that commissioning nurses are making locally.  It also makes recommendations for national organisations and CCGs themselves on how they can support the commissioning nurse to be as effective as possible.

Nursing Associate role
Health Education England (HEE) has published the response to its consultation on the Nursing Associate role.  This report outlines HEE’s response to the feedback received on “Building capacity to care and capability to treat - a new team member for health and social care: Consultation” and identifies how HEE will take forward proposals to introduce a new nursing support role in England by Spring 2017 to support the Registered Nurse workforce in providing high quality care across health and social care settings.

Unison Report
Pushing the call button on unsafe staffing: who will come to our aid?

Unison presents the results of a UK-wide annual survey of nursing professionals and it finds that almost 63% of respondents said that they felt there were inadequate numbers of staff on the wards to ensure safe, dignified and compassionate care; 67% believed that staffing levels had deteriorated since last year's survey; and 68% had considered leaving their role in the previous year.

Reshaping the workforce
The Nuffield Trust has published Reshaping the workforce to deliver the care patients need.  This report examines how best NHS staffing can be reorganised to support new ways of delivering care to patients. It finds that equipping the existing non-medical workforce, NHS nursing, community and support staff with additional skills is the best way to develop the capacity of the health service workforce.  The paper provides practical guidance on implementation to those wishing to reshape their workforce, and identify opportunities to ‘grow’ the workforce.
End of life care

Addressing Inequalities
The Care Quality Commission has published a suite of documents: A different ending: addressing inequalities in end of life care.  These have been produced as part of the CQC end of life care thematic review and are designed to be read in conjunction with each other. The review looks at ‘how dying patients are treated across various settings’.


End-of-life care resources
NHS Employers has developed a range of information and tools to train staff in end of life care.  The resources include: an overview of end-of-life care including where the strategy has come from and what work has been done nationally; benefits of investing in training including how it can build a skilled workforce for the future; case studies from NHS organisations; a list of e-learning resources and training programmes; and tools to implement an end-of-life care strategy in organisations.


Government Committment
The Department of Health has published Our commitment to you for end of life care:  the Government response to the review of choice in end of life care.   This report details the six commitments that the government has made to the public to end variation in end of life care across the health system by 2020.

End-of-life care case study
NHS Employers has published a case study from Salford Royal NHS Foundation Trust describing how staff are empowered to deliver end-of-life care.  Making staff experts in delivering end-of-life care illustrates that through investing in education and training, the trust has been awarded an outstanding CQC rating for its end-of-life care services;  improved patient care through improved staff communications; and received excellent feedback from patients' families about the service provided. 


End of life care for LGBT people
Marie Curie has published Hiding who I am: exposing the reality of end of life care for LGBT people. This report looks at the barriers that prevent lesbian, gay, bisexual and trans (LGBT) people from accessing end of life care and highlights their real-life experiences.  It draws on interviews with LGBT people living with a terminal illness, and their partners. Many people interviewed said they felt anxiety about having to hide who they are when accessing end of life care, with one in four experiencing discrimination from health and social care professionals in their lifetime.

Five new animated educational films launched for professionals
These short animated films cover topics including:
•    Discussing dying
•    Talking to children who are bereaved
•    Discussing Adult Authorised (Hospital) Post Mortem Examination
•    Dealing with unsuccessful neonatal resuscitation
•    Understanding the processes following a sudden or unexplained death

Innovative nursing assistants programme
The Social Care Institute for Excellence has published Evaluation of the Care Assistant Development Programme: learning from early implementation. This programme aims to develop senior carers into nursing assistants who are trained up to assist with medication and care planning. 


Tackling bullying and harassment in the NHS
NHS Employers Health, Safety and Wellbeing Partnership Group (HSWPG) has launched a set new resources and an infographic to support NHS organisations to develop cultures where staff are free from the fear of intimidating behaviour.  The new resources also include a webpage containing practical tools, evidence and support for tackling bullying.

All Our Health
Public Health England has published a presentation delivered by the Chief Nurse to inform healthcare professionals about All Our Health, a call to action to healthcare professionals working with patients and the population to prevent illness, protect health and promote wellbeing. 
 
Providing nursing care for trans people
The Royal College of Nursing (RCN) has published Fair care for trans people: an RCN guide for nursing and health care professionals.
