Good evening 5th Ward,

As a friendly reminder, the 5th Ward Service Office will host the Department of Public Health tomorrow, October 6, 2015 for an influenza walk-in clinic from 11:00 A.M. - 3:00 P.M. Please bring your medicare or insurance card, if you have one.

The 5th Ward Service Office is coming to a location near you. The 5th Ward Mobile Office was so popular last year, we decided to do it again. Look for us tomorrow at Treasure Island, (1526 E. 55th St.) from 11 A.M. - 1 P.M. in the community room. There will be someone there to answer your questions, take service request and share literature on services that our office provides.
