Region 6 09/08/2015 - 3 oil well service workers killed in west Texas rig fire due to ignition hazard and lack of flame-retardant clothing, OSHA finds
Region 6 09/30/2015 - 36-foot fall kills construction worker in New Braunfels, Texas; OSHA cites contractors
Region 7 10/15/2015 - OSHA finds improper load rigging caused fatal worker injury in Lincoln, Nebraska
Region 7 10/07/2015 - Safety mechanisms would have prevented worker's death

And the list goes on and on and on.... It is very easy to see that each of these fatalities could have been prevented. Unfortunately we can not go back in time to reverse the devastating affects, but we can influence the future if we take the time to slow down and understand what we are trying to accomplish and ensure we do it the right way the first time.
