“Scratch Your Head and Punt"


Patient referrals come from a variety of sources; past patients, doctors, friends, etc. Over the past few years, I have been receiving an increasing number of referrals from you . . . my colleagues. When a patient presents to you, as a therapist, and after multiple attempts, you are scratching your head, you have been gracious enough to refer them to me. I thank you and appreciate your confidence and I also realize the heat some of you take for referring outside your practice.  It is after all, the ethical and compassionate thing to do. 

A few weeks ago a 51 yr. old male patient was referred by a physical therapist (actually after having seen two therapists at different practices) within 3 - 4 months for progressive pelvic floor pain

Q:  Hi Tom,  Long time MET user, looking for the missing piece to get this gal over the hump. Pt is a 42 yo obese female with right SI and sacral pain for about 1 year.  Pain started after attempting to train for a 5K.  She has a left on left sacral torsion.  Initially had innominate/pubic dysfunction which is no longer there.  She is difficult to palpate due to body habitus but I do not think she has a lumbar dysfunction at this time either.  The torsion will reduce with MET but I cannot get it to fully correct.  She does have some piriformis and psoas tightness that we continue to work on.  Saw a DO twice who agreed with this assessment and states he was able to improve but not completely resolve the torsion.  Has had an epidural steroid injection at L4-5 and a right SI injection.  Does have positive MRI for L5-S1 tiny disc protrusion.  Responding well to extension based exercise.  HEP currently includes transverse abdominus isometrics, piriformis and psoas stretching, right trunk rotation, and prone press ups.  Pain abolishes with the combination of prone lying, Unilateral PA mobilization on (L) sacral base, and a cephalad glide of the (L) ILA.  Any input you may have is greatly appreciated.

A:  A wild guess but . . . The sacral asymmetry may have appeared to not completely resolve but it may have been a new pattern. Example: it was a L on L (R suclus deep) but maybe it was a L on R (L sucus back) after the initial correction. It would look the same unless you motion test again.  Might also explain why the piriformis does not calm down. Just one hunch, treat for a L on R backward torsion and see how she does and please let me know how you make out.

Muscle Energy Technique is one of the most effective and valuable manual techniques for the identification and correction of somatic asymmetry and hypomobility.
