Have you been discharged from a Community Mental Health Team (CMHT)?
We would like to hear from anyone who has at any time in the past been discharged from their CMHT to find out what the experience was like and whether you felt it helped you or not.
Discharge is an important part of all health and social care services, but it should not mean the end of services. We want to try to find out how the process works so that we can work on making it better.
We do not need to know any personal information and will not be passing on comments to anyone else unless you would like us to. We are hoping to use what we find to develop some new ways of getting information out to people so that others going through the process will be prepared. We would also like to know what other types of support you might have relied on during that time that helped you.

SAVE the DATE - Vale of Glamorgan Summer Event

West Vale CMHT - Updated phone & fax numbers
The West Vale Vale Community Mental Health Team provides a specialist service for adults aged 18 to 65 years who experience severe, enduring and complex mental health difficulties and who are residents of the western side of the Vale of Glamorgan or registered with a GP surgery in our catchment area.
With effect from Tuesday 16 February 2016 the fax number for the West Vale CMHT will change to 01446 776320.
The recently changed telephone number is unaffected and remains as 01446 776300. We would be grateful if you were to discard any other numbers as of that date.

Innovate Trust
Originating from a student volunteer project at Cardiff University, Innovate Trust provides support and guidance to disabled people. Our main work is to support people with learning disabilities, but we also support individuals with mental health issues and people with physical impairments. In addition we provide support to elderly, young, disadvantaged and vulnerable members of the local community through our Student Volunteer projects still based at Cardiff University. Innovate Trust Community Living
Our personal care services provided by the agency include support for ensuring that the individual service user receives an individualised package of care assessed and commissioned by social services.
These consist of assistance with all, or some daily functions i.e. getting out of bed , washing, dressing, eating, keeping warm, taking medication, assisting in the management of continence, ensuring the person is supervised in terms of inappropriate behaviour, and social activities.
Some of this assistance may be directly given by the care worker/s depending on the individual need of the service user or it may involve more independent service users being prompted to undertake the tasks of daily living with minimal direct assistance. The care provision may be targeted i.e. at certain times of the day or on a 24 hour, 7 day a week basis depending on assessed need.
The agency also offers other support which falls outside the definition of personal care i.e. general counselling and support such as:
- Assistance with maintaining the security of a dwelling
- Giving advice on a range of issues in terms of servicing or using appliances.
- Helping a person comply with e.g. terms of a tenancy agreement, help with budgeting, finances and benefits
- Maintaining positive relationships with families, friends, neighbours etc
- Support with any other personal, clinical or medical conditions and their treatment including, keeping appointments, attending for regular check ups e.g. for GP, dentist, hospital other specialist medical, sensory, citizenship, legal support or interventions.
