Have you been discharged from a Community Mental Health Team (CMHT)?
We would like to hear from anyone who has at any time in the past been discharged from their CMHT to find out what the experience was like and whether you felt it helped you or not.
 
Discharge is an important part of all health and social care services, but it should not mean the end of services. We want to try to find out how the process works so that we can work on making it better.
 
We do not need to know any personal information and will not be passing on comments to anyone else unless you would like us to. We are hoping to use what we find to develop some new ways of getting information out to people so that others going through the process will be prepared. We would also like to know what other types of support you might have relied on during that time that helped you.

West Vale CMHT - Updated phone & fax numbers
The West Vale Vale Community Mental Health Team provides a specialist service for adults aged 18 to 65 years who experience severe, enduring and complex mental health difficulties and who are residents of the western side of the Vale of Glamorgan or registered with a GP surgery in our catchment area.
 
With effect from Tuesday 16 February 2016 the fax number for the West Vale CMHT will change to 01446 776320.
 
The recently changed telephone number is unaffected and remains as 01446 776300. We would be grateful if you were to discard any other numbers as of that date.

Innovate Trust
Originating from a student volunteer project at Cardiff University, Innovate Trust provides support and guidance to disabled people. Our main work is to support people with learning disabilities, but we also support individuals with mental health issues and people with physical impairments.  In addition we provide support to elderly, young, disadvantaged and vulnerable members of the local community through our Student Volunteer projects still based at Cardiff University. Innovate Trust Community Living
Our personal care services provided by the agency include support for ensuring that the individual service user receives an individualised package of care assessed and commissioned by social services.
These consist of assistance with all, or some daily functions i.e. getting out of bed , washing, dressing, eating, keeping warm, taking medication, assisting in the management of continence, ensuring the person is supervised in terms of inappropriate behaviour, and social activities.
 Some of this assistance may be directly given by the care worker/s depending on the individual need of the service user or it may involve more independent service users being prompted to undertake the tasks of daily living with minimal direct assistance. The care provision may be targeted i.e. at certain times of the day or on a 24 hour, 7 day a week basis depending on assessed need.
 The agency also offers other support which falls outside the definition of personal care i.e. general counselling and support such as:
  • Assistance with maintaining the security of a dwelling
  • Giving advice on a range of issues in terms of servicing or using appliances.
  • Helping a person comply with e.g. terms of a tenancy agreement, help with budgeting, finances and benefits
  • Maintaining positive relationships with families, friends, neighbours etc
  • Support with any other personal, clinical or medical conditions and their treatment including, keeping appointments, attending for regular check ups e.g. for GP, dentist, hospital other specialist medical, sensory, citizenship, legal support or interventions.

FREE Agored Training on BME Mental Health

This is an accredited course.

  • Understand the extent of mental health issues in the Black and Minority Ethnic (BME) population
  • Understand mental ill health in individuals from BME communities
  • Understand how to signpost individuals from BME communities to mental health services

To receive the accredited qualification delegates will need to complete an assignment relating to the topic following the course.

Care & Repair Cardiff and the Vale
New Warm and Well Project
Fuel poverty has a significant impact on the health and social and economic well-being of older people living in Cardiff and the Vale of Glamorgan. Older people are particularly vulnerable to the effects of poor health linked to fuel poverty and living in a cold home. As well as causing illness and making existing health conditions worse, inadequate warmth in an older person’s home can affect physical dexterity and subsequently increase the risk of falls. There is also a link between fuel poverty and the number of excessive deaths amongst older people during the winter months.

The Warm and Well Project aims to provide advice and support to older people whose living conditions are impacting negatively on their health. The project is looking to work closely with local health, housing and social care professionals in order to support and assist people aged 60 and over who live in Cardiff and the Vale of Glamorgan who have a health condition or illness that has been caused or made worse by their living conditions.

The Warm and Well Project will enable frontline workers within the health, housing and social care sectors to refer individuals to the project if they identify somebody with a cold related illness and / or if another factor is identified.

The Warm and Well Project can provide a range of services, including:                             
  • support with heating repairs and improvements
  • support with general repair and maintenance works that could be undertaken to improve the energy efficiency of their homes
  • assistance to access grants, charitable / benevolent and / or other funding for works
  •  advice and information on more appropriate ways of purchasing fuel and energy
  • advice and information on energy saving measures and tips that can be undertaken to save energy in the home 
  • income maximisation and benefit advice to identify any unclaimed benefits
  • falls prevention advice
  • advice on healthy eating, smoking cessation, alcohol consumption and exercise

Living with ADHD, Working together to overcome the challenge...
Attention Deficiency Hyperactive Disorder affects between 2 to 5% of children in the UK . Common symptoms of ADHD include:
  • a short attention span or being easily distracted
  • restlessness, constant fidgeting or overactivity
  • being impulsive
Unless it is recognised and treated a child’s life can be blighted.

Cardiff & the Vale Parents’ Federation is hosting an event where we shall have presentations from the following speakers 
  • Sharifah Ahmed—researcher Cardiff University - explains how recent research findings have shed light on genetic links to ADHD 
  • Eleri Probert—Cardiff & Vale University Health Board - how the Wales NHS’ new Neuro Developmental Disorder Service may be able to help you
  • Marsi Kou - Living with ADHD from an adults perspective 
  • Carly Vella - (SNAP) support with Education and Schooling
Presentations will be followed by a plenary where we can discuss what support families need and how we can help to set up this support. Information for families will be available on the day.
