Are Older Adults in Your Region Getting Patient-Centered Care?

The new Dartmouth Atlas, funded by The John A. Hartford Foundation, is a report card that analyzes Medicare data to show us where the United States is making progress in patient-centered, evidence-based care for Medicare beneficiaries and where improvement is still needed. It also offers insight into regional variations in care. Visit our special “Our Parents, Ourselves” webpage for a PDF of the report, key data tables, our Health AGEnda blog post, and media coverage.

First Two John A. Hartford Foundation Change AGEnts Issue Briefs Address Dementia Care, Elder Justice

The John A. Hartford Foundation Change AGEnts Initiative has produced a series of seven Issue Briefs that represent a summary of intensive working group meetings that took place at the Change AGEnts annual conference in December. The Issue Briefs are intended to inform future work to create widespread and systemic changes in the care of older adults. Read the first two briefs on Dementia Care and Elder Justice.

The other issue briefs will be published on our Health AGEnda blog in the coming weeks.
