And, a Blue CareOnDemand visit is less expensive for employers than primary care, urgent care or emergency room visits. If your employees choose Blue CareOnDemand (when appropriate), you may save money!

What is the cost?
If you choose to offer this program, there are no additional administrative fees. However, your plan will be charged $59 per visit. We can work with you to establish your member cost share. Please note that if you have a high deductible health plan, your employees will pay $59 until they reach their deductible.
What do you need to do?
If you would like to offer Blue CareOnDemand to your employees, complete this form and send it back to your BlueChoice® account representative. You can add this service at any time. You do not need to wait for your renewal.
