Self-Insured Accounts

Blue CareOnDemand is now available to BlueChoice HealthPlan self-insured accounts. BlueChoice^® will email this letter to all of our self-insured accounts to see if they would like to offer the service to their employees. If so, they will need to complete this form and return it to their BlueChoice representative.
