Pharmacy HIT Collaborative Meets with CMS and ONC about Pharmacists involvement is State Medicaid HIE initiative
In March, the Centers for Medicare & Medicaid Services (CMS) issued guidance on promoting State Health Information Exchange (HIE) programs by matching 90 percent funds to support the coordination of care and transitions of care requirements related to Medicaid EHR incentive payments to connect other Medicaid Providers including pharmacies and pharmacists. 
According to Dr. Tricia Lee Wilkins (Pharmacy Advisor and Health IT Specialist, ONC), “this is an opportunity to support state level efforts for the “initial on-boarding of pharmacies to HIEs or other interoperable systems. The goal is to improve coordination of care between providers and across settings of care.  As states consider ways to enhance their health IT infrastructure, please think of ways your organization can partner/collaborate with states to enhance data exchange and connectivity with pharmacies.“
Pharmacy HIT Collaborative Request SNOMED CT and Value Sets
Discover the pharmacy SNOMED CT codes and value sets. Visit the Value Set Authority Center. To access the tool, you’ll need a Unified Medical Language System (UMLS) account through the National Library of Medicine. Don’t worry!  It’s free and everyone can get access.  When you search VSAC, look for value sets stewarded by “Pharmacy HIT” – that’s us.  If you don’t find what you are looking for, you can submit a question to us, request a new SNOMED CT code, or request updates to Value Sets.
Pharmacy HIT Collaborative’s CMS Comments to Methodological Changes 2017 Part D Reporting Requirements for MTM
On March 4, 2016, the Pharmacy HIT Collaborative submitted comments for the Advance Notice of Methodological Changes for Calendar Year (CY) 2017 for Medicare Advantage (MA) Capitation Rates, Part C and Part D Payment Policies and 2017 Call Letter. Our comments pertain to the call letter’s Part D Reporting Requirements for MTM. The Collaborative has been working with several pharmacy organizations and other groups regarding the use of a structured coding system – SNOMED CT – for medication therapy management (MTM) services clinical documentation by pharmacists. We believe that the consistent use of structured universal codes is critical to the expansion of documentation of MTM services and support the use and implementation of SNOMED CT codes for these services.
Pharmacy HIT Collaborative’s ONC Comments to 2016 Interoperability Standards Advisory
On March 21, 2016, the membership of the Collaborative submitted a comment letter and table regarding the proposed final 2016 Interoperability Standards Advisory. The Collaborative has been involved with the federal agencies, including the Office of the National Coordinator (ONC), developing the national health information technology (HIT) framework since 2010.
NCPDP Electronic Structured Documents for Pharmacists Clinical Data Exchange
National Council of Prescription Drug Program (NCPDP) Work Group 10 has developed a document to provide guidance to the pharmacy sector on the use of the HL7 Consolidated CDA (C-CDA) template document format as the vehicle for capture and exchange of the pharmacist clinical information through HIE's. Content of two document formats are included: Transition of Care Between Inpatient and Community Pharmacy and Pharmacy/Pharmacist Care Note (PCN). In addition, there is a standard electronic version of the CMR implementation guide available for vendors to program HL7 CDA® R2 Implementation Guide: Medication Therapy Management (MTM) Templates, Release 1 - US Realm. This would allow an electronic version of the CMR to be transmitted in a standard way through an HIE to the PCP or other providers.
Pharmacy HIT Collaborative’s mentioned in APhA’s 2015 Annual Report
According to the APhA 2015 Annual Report, the Pharmacy HIT Collaborative works to ensure that HIT supports delivery of health care services and that pharmacist can electronically exchange information. The HIT Collaborative also uses templates to standardize and align patient’s information within the larger healthcare system. In 2015, the Collaborative published many valuable coding and guidance documents for the profession.
