New Guide Released - Chronic Care Management in a Patient’s Medical Home
The CFPC’s Best Advice guide
Chronic Care Management in a Patient’s Medical Home, provides actionable advice to family physicians about chronic care management in family practice settings. While this guide applies to all types of family practices, the strategies described involve many important components of the Patient’s Medical Home model. With proper intervention and ongoing care, family physicians can help prevent and control the effects of chronic diseases by implementing practical chronic care management strategies in their practices.

Better Home Care: A National Action Plan
As part of a new multi-year Health Accord, the federal government intends to work with provincial and territorial governments in their efforts to make home care more available. Better Home Care: A National Action Plan will provide clear direction and achievable goals to make this happen.

The CFPC, the Canadian Home Care Association (CHCA), the Canadian Nurses Association (CNA) have partnered to spearhead the Action Plan. Consultations will include invitational policy roundtables, surveys and calls for innovation. Add your voice to the consultations by filling out the survey.

This year marks the 150th anniversary of parliamentary buildings on Parliament Hill in Ottawa.

To commemorate this milestone, MPs and senators from the current Parliament gathered on the steps in front of Parliament Hill for a rare group photo.
