How I Treat Piriformis Syndrome


I get this question a lot and I thought it might be fitting for the first “How I Treat” clinical pearl. Regardless of the actual problem, many diagnosticians still call it Piriformis Syndrome, which is fine by me, but it is very much a statement of just symptoms.  I try to look at the entire picture, which involves a full structural look at the pelvis and the sacrum, as well as the lumbar spine. I tend to reject the notion that the problem starts out in the Piriformis muscle and instead, look for other causes that make the Piriformis react / adapt.

Whatever I find, be it a pelvic, sacrum or lumbar spine issue, I make sure to:
  • Use Muscle Energy Technique to try to get the pelvis, sacrum and lumbar spine level and symmetrical. This usually gets functional leg length back to equal as well.
  • Next I utilize counter-strain for the two main trigger points that usually implicate the piriformis.  See Treatment Protocol

Q:  Hi Tom, I have used your procedure many times with great success to correct pelvic misalignments which I learned from your excellent videos. A problem I have encountered is when checking the pubic bone with people who are a bit overweight getting to the pubic bone to check its alignment can be difficult and uncomfortable for the client.  Do you have a solution?

A:  Hi Kevin, Yes there is a trick to make palpation less . . . how shall we say . . . invasive. Gently take the patient’s hands and have them help you by holding the excess tissue up toward their chest and out of your field of palpation. Just tell them you need their help and they are usually very happy to comply.

17 of the most specific, bizarre ICD-10 codes


With 55,000 new codes, ICD-10 is much more specific than its predecessor. How fine-grained? Well, one code for suturing an artery will become 195 codes, and there are 312 animal codes alltogether. Beyond that, some codes, most of which specify cause or location, seem downright nutty — borderline unbelievable even. Here are some of the most bizarre:
  1. Z63.1 Problems with the in-laws - Put the whole family in one room during the holiday season and things can get ugly.
  2. T71.231D Asphyxiation due to being trapped in a discarded refrigerator, accidental - I don’t care how much you love your Kenmore Elite; when it’s time to discard it, let it go.

The following exercises should be approached with extreme caution for baseball players, other overhead athletes, and anyone with shoulder pain. For baseball pitchers these exercises should almost never be performed. I am sure some professionals may still recommend these exercises for overhead athletes, but in my experiences they are much more likely to contribute to both shoulder and elbow injuries than prevent them.

Muscle Energy Technique is one of the most effective and valuable manual techniques for the identification and correction of somatic asymmetry and hypomobility.
