Addressing HIV Care and Housing Coordination through Data Integration to Improve Health Outcomes along the HIV Care Continuum Funding Opportunity

Purpose: The purpose of this three-year cooperative agreement is to support a single organization that will serve as the Coordination and Technical Assistance Center (CTAC) to provide technical assistance and capacity building (TA) to a minimum four (4) performance sites to implement and evaluate the electronic integration of housing and HIV care and treatment data systems to better coordinate service delivery and enhance patient navigation to improve health outcomes along the HIV Care Continuum. Recognizing the role of housing in supporting Persons Living with HIV (PLWH), this initiative seeks to support the electronic integration of housing and HIV care data systems in the coordination of housing and HIV care services. The CTAC will select and fund the performance sites through subawards. The CTAC will provide technical assistance (TA) to the performance sites in the design and implementation of the integrated data systems. In addition, the CTAC will conduct an evaluation of each integrated system and how it affects the provision of clinical practice in terms of provider acceptance, utilization, and in the coordination of housing and HIV care services to improve health outcomes along the HIV care continuum.

Funding: Approximately $1,200,000 is expected to be available annually to fund one (1) awardee. Applicants should note that the award amount includes annual subawards of $175,000 each to at least four (4) performance site locations.

The Health Resources and Services Administration's (HRSA) HIV/AIDS Bureau (HAB) will host a technical assistance (TA) webinar for interested applicants on Thursday, June 18, 2015 from 2:00 – 4:00 PM EDT (HRSA-14-055). The purpose of this webinar is to assist potential applicants in preparing applications that address the requirements of this funding announcement.
