Prescription Endorsing

Do you want to know how to endorse and submit your prescriptions correctly without over-endorsing, because you’re not quite sure of what to include?

Declaration of competence
(DoC) is a system of self-assessment that is already in use for the New Medicines Service. CPPE have now extended the DoC to cover commissioned services and have received support from Health Education England, NHS England and Public Health England to use this nationally. Julia Booth, Primary Care Contracts Manager, NHS England South, has shared some analysis of the MUR quarterly returns.  The target of 50% of consultations being in target groups has been exceeded in all areas however whilst that overall target for 2013-14 was exceeded, the proportion of MURs completed for patients recently discharged from hospital was exceptionally low. This will be an area of work for the LPN to develop going forward and will require collaborative working from community pharmacy and secondary care.

Medicines optimisation is about ensuring that the right patients get the right choice of medicine, at the right time and can help encourage patients to take ownership of their treatment. NHS England released a new medicines optimisation dashboard on May 22nd, to help CCGs to understand how well their local populations are being supported, to optimise medicines use and inform local planning. For more information have a look at the NHS England website.

Remember to encourage needle exchange clients to return used equipment when collecting new supplies.
The Royal Pharmaceutical Society is in the process of updating its specials guidance, to be published online in due course.

Since April 2015, the Renal department at Dorset County Hospital stopped routinely giving/sending FP10’s to patients to obtain their immunosuppression medication from their local community pharmacy as they have been going through a homecare provider. If a patient needs an urgent dose change or supply, FP10s will still be used. The drugs mainly affected are tacrolimus (Adoport), mycophenolate mofetil, mycophenolic acid (Myfortic), sirolimus (Rapamune) and ciclosporin (Neoral). Patients were sent a letter with an accompanying patient information leaflet.  Contractors may wish to run down their stocks.  For any queries please liaise with the Renal Outpatient Pharmacist via RenalOutpatientPharmacist@dchft.nhs.uk.

Direction of prescriptions FAQs
PSNC has published an update for contractors along with advice on what to do if they are affected by this issue.   

As part of work to address the issue NHS England will soon be publishing a poster designed to increase public awareness of their choice of service. The  poster, which will be sent to GPs and pharmacists, is to inform patients that they are free to choose any pharmacy they would like to use for the dispensing of prescriptions, and in doing so to reduce the direction of prescriptions.

PSNC will continue to monitor the situation and will seek regulatory change if direction of prescriptions continues to be a problem. In the meantime the FAQs document sets out the steps contractors can take if they are affected by the issue. It includes resources to use to contact patients and information about recent changes to the Pricing Authority’s data publication policies which mean that pharmacy level dispensing data, which can provide compelling evidence of the direction of prescriptions, is available online.
Dorset LPC recently held an educational event, the Management & Treatment of Irritable Bowel Syndrome. This was led by Sue Surgenor, Inflammatory Bowel Disease Consultant Nurse Specialist at Poole Hospital. A copy of the presentation from June 9th is available on the Dorset LPC website.   Jo Cooper (07775 898224) of Almirall is happy to support local pharmacies with mini IBS training sessions to support Sue's talk.

National support for local commissioning
A working group of the national bodies (PSNC, AIMp, CCA and NPA) and representatives of LPCs is to progress the work on national support for local commissioning. As previously reported  a working group is to be formed to prepare proposals for a national provider company that would also provide support for local provider companies.  The working group will prepare proposals for the boards of the national bodies to consider in early autumn. More details will follow as they become available.
Local Authority Flu Vaccination
At a recent meeting with Public Health Dorset the 2014-15 flu vaccination service for Local Authority employees was discussed. Public Health Dorset thanked the LPC and contractors for their prompt responses to the very short timelines for getting the service commissioned. They also wished to thank contractors for their delivery of the service itself.

There is now an improved prescription tracker which allows checking the status and history of electronic prescriptions. Contractors can now search by the NHS number, date range and prescription status and the search will show the nominated dispenser.  However contractors must have a legitimate reason to verify that a prescription exists or to identify a prescription status. GP practices in the county of Dorset  are now actively moving towards more use of repeat dispensing (RD) and it is important that pharmacists and GPs work together to deliver the benefits of the service. Implementing the RD service is an essential service and all pharmacies are required to be able to process these scripts.
