Revisions to CHW Coverage Guidelines for Minnesota Health Care Programs (MHCP)

With input from the Alliance and the Diabetes Unit at the Minnesota Department of Health, the Minnesota Department of Human Services has raised the current monthly cap on diagnostic-related patient education services delivered by supervised CHW certificate holders to reflect CHW-led group education needs. This is a major step in the right direction!

Register NOW for CHW-Community Paramedic Learning Communities Kick-Off on 9/24/15 and Fall Learning Sessions

CHWs, supervisors, and stakeholders are all invited to participate in the SIM Emerging Professions Learning Community (SIM-EPLC). This fall the SIM-EPLC will bring CHWs and Community Paramedics (CPS), their health care teams, and a broad network of stakeholders together for the first time in a series of interactive events and dialogue around critical issues facing both emerging fields. By sharing our respective expertise and addressing current challenges, we can streamline collaboration, build upon best practices, and create new solutions to barriers while improving the access, quality, and impact of our health care response through better patient-centered care.

The first event is a kickoff on Thursday, September 24th, 2015, at the Minneapolis Urban League. All events will have webinar access for those unable to attend in person. Please consider attending, sending your team, and sharing this great opportunity with your networks.

"Silos to Circles" Free Dinner Meeting on 9/24/15 - CHWs encouraged to attend!

You are invited to join Silos to Circles, a cross-continuum health coalition, on Thursday, September 24th from 6:00-9:00 p.m. for Access!: Catalyzing Community Capacity for Health and Well-being, a feast and dialogue.

During our time together we will explore how to promote person-directed health and well-being within the context of chronic disease and behavioral health. Pioneered by Marnita's Table, Silos to Circles will be convening in the model of Intentional Social Interaction (ISI) in partnership with the Citizens League. We are inviting you because we believe that you will add value and insight into this sensitive and timely topic. If there is someone else you know who should be a part of this dialogue, please feel free to extend an invitation for us. This event is being held by the following community allies: Catalyst Initiative of the George Family Foundation, Marnita's Table, Citizens League, and Silos to Circles. Teens and children are also welcome to attend.

Provide Testimony at 9/28/15 Senate Committee Hearing on Health Disparities

Lift your voice on the role of CHWs in reducing health disparities at this meeting chaired by MN State Senator Kathy Sheran at 5:30 p.m. at the Center for Changing Lives, 2400 Park Avenue, Minneapolis, MN.

The Committee on Health, Human Services, and Housing is seeking community member input on reducing health disparities. Please contact Chelsea Magadance at chelsea.magadance@senate.mn if you would like to testify or for more information.
