What is HomeBASE?   How did it come to be?
 
In 2011-2012, a DOC resident worked on a project with some data showing that DOC patients went to the ED more often than other patients in the health system.  That data lead to “DOC Redesign,” an effort to change how we do business in order to take better care of our patients.  HomeBASE was born in 2013 as phase 1 of DOC Redesign.   
 
HomeBASE builds on many different coordinated care models.  The goals of HomeBASE are to reduce ED and inpatient use AND to get our patients better connected with their PCP.   The HomeBASE team is guided by the principle that frequent ED use often means that the patient has unmet psychosocial needs. Patients may be homeless and/or be using substances to cope with mental illness or physical symptoms.   They may also need help promoting their own health and navigating our complex healthcare system.
