Program news

Welcome Stacy Martens!

Please join us in welcoming Stacy Martens as the new program assistant supporting the WWC and WISEWOMAN programs. Stacy started mid-September, transferring from the Colorado Department of Labor and Employment where she has served as a rehabilitation counselor and program assistant in the Division of Vocational Rehabilitation. Stacy looks forward to sharing her years of experience supporting fellow Coloradans in reaching their healthiest potential. She can be reached at stacy.martens@state.co.us or 303-692-2998.

October 19th WWC Health Improvement Team (HIT) Call

The WWC HIT Call is October 19, 2017 from 11:00 a.m. to 12:00 p.m. During the October HIT call we will focus on the lawful presence requirement. We will discuss recent changes to the tools used to verify lawful presence as well as other changes and common questions regarding lawful presence. We will also review use of additional funding for allowable supplies/operating and travel purchases. WISEWOMAN agency staff members are encouraged to join on for the lawful presence portion of the call.

Century Link will prompt you to provide a phone number to join the webinar, however you will still need to call 1-877-820-7831 and enter pass code: 335241# (Press # after the pass code) for audio.


Remember to Use FY18 Invoice Template!

Please remind the person who completes invoices at your agency to use the updated FY18 invoice template provided to you. These updated templates include information which allows our fiscal officer to quickly (and correctly!) process your invoice for payment. 

How can you tell if they're using the new template you ask? Our fiscal officer, Cathy Van Tassel's name and contact information is in the "To be signed by CDPHE Fiscal Officer..." box. If we receive invoices on the old template, we'll be sending them back to agencies for correction. Please contact your program coordinator if you need a copy of the updated invoice template for your agency.


Progress Reports

There are no progress reports due for clinical services, care coordination and targeted outreach in October. Progress reports will be due in December, 2017.

CCGP Palm Cards are available for order in English, Spanish, and Vietnamese. Please email cdphe_wwc@state.co.us before October 11, 2017, to request additional cards. Please include the amount needed of each language.

Upcoming DTTAC Lifestyle Coach Training

Location: Colorado Department of Public and Health Environment, 4300 Cherry Creek Drive South, Denver, CO 80246
Dates: October 17 - 18
Time: 8:30 a.m. – 5:00 p.m. (both days)
Fee: $750 per person* 
*This is an included expense on the WISEWOMAN Allowable Purchase List.

The Colorado Department of Public Health and Environment and the Diabetes Training and Technical Assistance Center (DTTAC) at Emory University are hosting Lifestyle Coach training for the National Diabetes Prevention Program. 

DTTAC Lifestyle Coach Training is a highly interactive, 2-day, in-person training with an expert DTTAC Master Trainer that provides your Lifestyle Coaches with the skills, knowledge and experience that they need to successfully facilitate the proven-effective lifestyle change program.

DTTAC Lifestyle Coach training:

  • Features an interactive, small group format with hands-on practice in group facilitation techniques
  • Includes detailed review of the yearlong National DPP curriculum
  • Offers continuing education credits for registered dietitians and certified diabetes educators
  • Is open to organizations that have pending recognition or will apply for CDC recognition and
  • Includes the most up-to-date information on the recognition process and the national program.

CVD Nurses Fall Learning Program

The Rocky Mountain chapter of the Preventative Cardiovascular Nurses Association will be holding their Fall Learning Program on Saturday, October 14. This training program will discuss methods for reducing CVD risk in patients with diabetes as well as information on Antiplatlet Therapy. Breakfast and lunch are provided during the training and CE’s are available.

Chronic Disease Video Resources  

The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) released new videos that offer support to people with diabetes, and steps they can take to manage their disease. The patient video series can be found here

The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length. The provider video series can be found on the partnership’s YouTube page

Immigrant Eligibility for Coverage Programs

Wednesday, October 4, 2017, 11:00 a.m. – 12:00 p.m.

This webinar will provide an overview of immigrant eligibility polices for health insurance affordability programs and will help explain how to navigate the application process for families that include immigrants. This webinar is presented through a partnership between the Center on Budget and Policy Priorities and the National Immigration Law Center.

Community Health Workers Training for Health Equity Webinar

The Achieving Health Equity Across Heart Disease and Diabetes Virtual Community of Practice will be hosting an upcoming webinar to address training and certification of Community Health Workers (CHWs) to support health equity. The webinar will be held on Tuesday, October 31 from 12 – 1:30 p.m. Programs from Massachusetts, Minnesota and Wisconsin will share their experiences working to train and certify CHWs working in health equity. While the webinar is targeted to chronic disease programs working with CDC grants, it is open to all organizations that utilize CHWs. Click here to register for the webinar.
