Patient Success Stories

" I was blind and now I can see. What happened to me was a miracle" - Jose
MyClinic patient Jose Guzman is rediscovering the gift of sight.
Jose Guzman is a local Jupiter resident who had been suffering from uncontrolled diabetes for most of his life without any knowledge of his own condition. Jose's wife, who was already a patient of MyClinic, encouraged her husband to seek help here. With the help of MyClinic volunteer providers and staff Jose was able to get his diabetes under control. Unfortunately Jose's vision had progressively worsened to the point where he was 80% blind in both eyes. Headaches, anxiety and frustration became part of his daily life. MyClinic's Care Coordinator, Patricia Ferrer, worked to find Jose a local ophthalmologist to volunteer his time and services to see Jose. Dr. Skolnick reassured Jose that his condition can be corrected through surgery. Like a new born baby absorbing his surroundings in pure wonder, Jose is rediscovering the gift of sight.

  • What is the economic value of our volunteers? This year, those individuals who dedicated their time, talent, and energy to making a difference at MyClinic will have donated over 5000 hours of volunteer service. In addition, Lab and Imaging tests are provided to MyClinic patients free of charge by Jupiter Medical Center. Donated time and services is valued at $500,000.

  • With over 2000 registered patients, MyClinic provides an average of 427 visits per month including adult and pediatric medical and behavioral health care, medical specialty care, referrals for urgent dental, and care coordination. Moreover, a total of 488 referrals for screening and diagnostic tests have been ordered this year. In addition to general physicals and routine medical issues, MyClinic provides ongoing care for chronic, potentially life threatening illnesses that were previously going untreated.

  • Demand for MyClinic services continues to grow with an average of 35 new patients registering each month. Exit survey results indicate that the majority of patients would have "gone without care" or "sought care at hospital emergency department" if MyClinic did not exist.

Diabetes is an epidemic of national proportions and state-level impact. The Sunshine State has teh second highest prevalence rate for diabetes in the nation. Innovative programs to address this disease and its consequences are increasingly critical. MyClinic and its community partners realize that the successful health systems of the future will be those that can apply integrated approaches to achieve improvements in health and, experience of care, and also reduce cost. Diabetes will be the first focus, as it disproportionately affects minorities. Through this initiative to improve population outcomes at scale, with a particular focus on reducing disparities we can convene learning networks to share best practices and proven approaches, and develop capacity within organizations for population health improvement.
