Dr. Sandra Delistathis, MD
Internal Medicine & Functional Medicine
We are pleased to announce the addition of Dr. Sandra Delistathis, MD, to our Functional Medicine and Primary Care team. Dr. Delistathis will join the team on Tuesdays, Thursdays, and Fridays starting April 4th.
Dr. Delistathis has been practicing medicine for the past sixteen years. Throughout her career, she has sought out more holistic approaches to medical care and is currently pursuing her certificate in Functional Medicine. She is very excited to join her practice with Capital Integrative Health in order to bring a balanced approach to the current practice of medicine.

DR. DELISTATHIS?
Joining our CIH membership is required for all patients participating in Dr. Delistathis's medical practice. For anyone interested in participating, we would like to offer you a one-visit medical consultation with Dr. Delistathis, MD, without having to pay the membership fee. This will allow you to see how her practice can benefit you. Follow the steps below to learn more and set up your appointment.
Here's what you need to know with this scenario:
Your visit would be billed to your insurance, and any co-pays, co-insurance or deductibles would apply
Labs, tests, prescriptions or supplements will not be ordered on the first visit
If you wish to continue in the practice with Dr. Delistathis as your provider, you will pay for the membership at checkout and schedule a follow up visit
Annual physicals are not applicable for initial consults

To schedule your first appointment with Dr. Delistathis, we ask you to do the following:
1. Email info@cihealth.org with the following information so that we can set you up in our patient portal. Please respond with the following information: full name, date of birth, gender, email address, phone number, and home address.
2. You will receive an email invitation to complete the patient portal registration. You will also be asked to complete a health history questionnaire.
3. Pay for membership here. (optional - remember, if you decide to pay for membership after your first appointment, please note that labs, tests, prescriptions and/or supplements will not be ordered on this first visit. After you decide if the practice and its approach is right for you, you will return for a second visit for testing and an outline of your treatment plan with Dr. Delistathis).
4. Call us at (240) 507-5110 to schedule your first appointment with Dr. Delistathis.
