After Hours activities in the SNPHN region

The Sydney North Primary Health Network (SNPHN) After Hours program is about improving access to primary health care in the after hours period so that people living in Sydney’s north can receive the right care, at the right time, in the right place.

As part of our response to improving access to after hours services for the community, we are working with health professionals and services to better coordinate and connect services, including the flow of information between service providers, to improve health outcomes. This article presents two examples of our after hours activities in the region. The first initiative is the GP Health Talks at TAFE and the second is around Extended Care Paramedics adopting Secure Messaging.

In this month's SNPHN News we bring you the following information:

• SNPHN is pleased to announce that 10 practices are participating in a locally developed Person Centred Medical Home program.

• SNPHN has been working with the Northern Sydney LHD on diabetes initiatives, including diabetes complications screening and training for practice nurses.

• The eligibility criteria for two of SNPHN’s commissioned dementia services has been expanded. 

• On Wednesday 8 February 2017, SNPHN held an End of Life Care co-design workshop. Learn more about this important palliative care initiative in our region.

• A reminder that TIS National provides a free interpreting service for private medical practitioners who have patients with limited English. 

• Hepatitis NSW Treatment Update Sheets provide an overview of the latest treatments for hep C.

• A letter from the Department of Veteran’s Affairs to GPs in regards to the Non-Liability Health Care program which provides for treatment of certain mental health conditions.

• Scholarships are available for NSW GPs in two new HETI courses: Psychiatric Medicine and Applied Mental Health Studies.

• GPs are invited to the Australian GP Alliance Congress 2017 on March 4.
