BlueChoice HealthPlan is pleased to introduce the
FOCUSfwd Wellness Incentive Program. This program offers tangible rewards to promote, motivate and incentivize eligible members to take part in improving and monitoring their health. Eligible members now have the chance to win cash-equivalent gift cards JUST for making healthy choices!
FOCUSfwd is included as part of our benefits package at no additional cost.
Who is eligible?
Focusfwd is available to active BlueChoice
® subscribers and their spouses who are ages 18 or older. This includes BlueOption
SM and BlueChoice products for individual, small and large group fully and self-insured members. Eligible m
embers who currently have the Healthy Rewards program will transition to the FOCUSfwd Wellness Incentive Program upon their group's renewal.
Why Change?
We reviewed our programs and realized they weren’t heavily utilized. We conducted extensive research to create a program that would benefit all of our eligible members. Our goal is to help them get healthy and stay healthy, which will lead to decreased medical costs and claims for groups.
How Does It Work?
Eligible members can access the
FOCUSfwd Wellness Incentive Program by:
- Registering for a free My Health Toolkit® account or logging in to their current account.
- Selecting the FOCUSfwd Incentive Program link under the Health and Wellness tab.
- Selecting Continue.
Eligible members will get 10 entries for logging into My Health Toolkit! When they are in the
FOCUSfwd site, they will see a list of the activities they can complete to earn more entries. They can sign up for Blue CareOnDemand, complete monthly wellness education, get a wellness exam, and more to earn entries!
Entries will accumulate throughout the year and increase their chances of winning the weekly, monthly and quarterly drawings AND winning the grand prize in December 2017!
