Welcome to this special survey issue of Disability Loop eNews!

We recently celebrated Disability Loop's second birthday. In the spirit of sharing, and getting wiser with age, we want to know more about you - and we want to give out some presents!

We have designed a survey to get to know you all better. Six random people will win a $50 Coles Myer voucher. 

The survey should take about 15-20 minutes to finish. We would really appreciate you taking some time to fill it in because your ideas will help us make Disability Loop even better and more useful for people with disability and their families.

We want to make sure we are here for you and are meeting your needs – both now and in the future.

What do you need? Tell us in our new survey

We have designed a survey that will help us understand what people with disability need - whether it comes from an NDIS plan, or somewhere else.

Your answers will help us to make sure we're giving you the best and most helpful information possible.

Who should fill the survey out? 

Anyone can fill out this survey. We designed it to be filled out by a person with disability, but we know our readers include families and friends, and other interested supporters. We'd like to hear from all of you. 

How big is the survey?

The survey has 30 questions, broken up into 5 sections. We think it could take about 15-20 minutes to finish. You can answer as many of the questions as you want. 

What if I get distracted and can't finish it? 

It's ok if you can't do it all at once - you can come back to it at any time and contiune where you left off - as long as you are using the same device (computer/smartphone/tablet), and the same browser (Internet Explorer/Safari/Chrome, etc).

5PM, Australian Eastern Standard Time

Will my answers be private? 

Yes. 

You do not have to give us your name or contact details unless you want to. 

If you want to be in the running for a chance to win one of six $50 Coles Myer gift cards, we will ask you to write your email address or other contact details in a box at the end, so that we can contact you if you win.

We also ask people if they'd like to subscribe to the Disability Loop eNews (this!), if they don't already.

If you do give us your contact details, we'll separate those details from the rest of the answers. This means that when we're looking at the other survey answers they won't be connected to anyone's personal information. Your answers will be private.    

What if I want to fill out the survey a different way?  

You can download an 
accessible word version (docx, 16 pages, 52kbs) of the survey if you prefer, or you can contact us by calling (03) 9662 3324, or via email at disabilityloop@afdo.org.au.
