New Video Released - The Patient's Medical Home: It Takes a Team
One of the pillars of the Patient’s Medical Home model is team-based care. An inter-professional team of health care providers working together for the benefit of the patient. The collaborative environment fostered by the Patient’s Medical Home brings together health professionals to offer a range of services to patients in response to the needs of the local community. Sioux Lookout First Nations Health Conditions
In December 2015, the CFPC wrote to federal health and Indigenous authorities in support of the Sioux Lookout First Nations Health Authority and their call for immediate action to rectify the alarming state of public health conditions confronting the First Nations communities of Northwestern Ontario. The CFPC highlighted the importance of improving access to primary health care providers as well as working with stakeholders to improve health conditions in remote, northern Indigenous communities.

It's promising to see provincial action on this topic with the Ontario government's
recent announcement. More can always be done. The CFPC continues to support Indigenous Health and will continue to produce documents such as our guide on systemic racism that will assist family physicians in delivering care to Indigenous patients. 

Prime Minister Justin Trudeau arrived in China on August 30th to begin his first official visit.
Canada and China have had a
long-standing relationship in the field of health. In 1995, the health ministers of both countries signed a Memorandum of Understanding for health cooperation which has since been reinforced by five Action Plans.
