Now's a Great Time —
Become a Viva Village Member!
Full membership in Viva Village is available to adults 18 and over who have need of the support and services Viva Village provides in order to remain in their homes.
- Be part of the 3-month Pilot and help us make sure our systems are working well. (As a bonus for being part of this Pilot, your membership will extend through December 31, 2017 — up to 15 months for one annual fee!)
or, if you prefer:
- Sign up to be a member starting in January 2017 and beyond.
Start your membership in one of three simple ways:
1) Call Viva Village: (503) 746-5082 and request a Getting To Know You form.
2) Email vivavillage@villagesnw.org with your name, home address, email address, phone number, and whether you want to be a Full Service Member or an Associate Member.
3) From the Viva Village website, download and print the Getting To Know You form at the bottom of the Membership page.
Mail your completed form to:
Viva Village
4905 SW Griffith Dr. Suite 108

The Next Steps:
You will be contacted to schedule an in-home interview with two members of the membership team. We can get to know you better and discuss how Viva Village can benefit you.
After a successful interview, you will receive a membership agreement, a payment form, and a questionnaire. Your membership begins once these have been processed.
