We need your opinion – EMPHN needs assessment general practice consultation
Eastern Melbourne PHN is seeking to understand the current needs of general practice. We encourage general practitioners, practice nurses and practice managers to complete a 20-minute survey that will add the general practice voice to our latest needs assessment of the region.
The survey focuses on what is driving demand, identifying issues in the referral process and providing context and insight into the issues we have previously identified. Our aim is always to best support providers through our programs.
Participants will not be asked for their names and their responses will be kept confidential. However, they will have the opportunity to provide their name if they wish to be involved in innovations and projects.
We are mailing a copy of the survey to general practices this week. Alternatively, the survey can be completed electronically via the following link: https://www.surveymonkey.com/r/EMPHNGeneralPracticeConsultation. The survey is open until close of business Monday 31 October 2016.

HealthPathways Melbourne (HPM) is commencing the formal review of the cardiology suite of pathways in October. GPs, specialists and community health providers are invited to provide feedback on the following cardiology pathways:
Please note that the Absolute cardiovascular risk assessment, Heart failure, Advanced or end-stage heart failure, and Heart failure rehabilitation pathways are not included in this review.
Service providers are encouraged to check their service details (phone, fax, clinic, times, referral forms) and let the HPM team know of any changes.

Austin Health embraces HealthPathways Melbourne
HealthPathways Melbourne has been positively received at Austin Health’s Grand Rounds. Dr Geoff Broomhall (HealthPathways Melbourne GP Clinical Editor) and Stuart Wilson (HealthPathways Coordinator) presented to Austin Health specialists to showcase the HealthPathways Melbourne website, the goals of the program in helping reduce avoidable hospital admissions and ED presentations and how Austin Health staff can participate in the pathway development process.
There was a lot of interest from various disciplines wanting to contribute to pathway development in particular respiratory, older persons and orthopaedics.
While work is in progress to expand HealthPathways Melbourne content, GPs in the North, West and Outer East can still access the following:
State-wide pathways such as Immunisation, Zika, Certificate of Death, Hepatitis C, Travel Vaccination, Advance Care Planning
Forms and resources including Digital Health, Organ and Tissue Donation, Multipurpose Taxi Program, Interpreter Services
Assessment and management advice across various conditions including Women’s Health, Respiratory, Cardiology, Orthopaedics, Gastroenterology and Diabetes.
