Welcome to the Compass Hospital Improvement Innovation Network (HIIN)!


Dedicated to continuous healthcare improvement, the Iowa Healthcare Collaborative's mission is to raise the standard of care, not only in Iowa, but nationally, through patient-centered initiatives to improve quality, safety and value.

IHC has been a proven champion of the Hospital Engagement Network (HEN) campaign since 2012. In the first three years, IHC HEN prevented more than 3,300 harms with an estimated cost savings of $50 million. IHC led Iowa hospitals from an education focused quality assurance model to an execution driven performance improvement model. Iowa's success was evident by the level of participation, uptake of best practices and progress made toward advancing the aims of the PfP.

IHC's Compass Hospital Improvement Innovation Network (HIIN) is designed to build on the successful HEN program to serve rural hospitals nationwide, using capacity to support innovative solutions targeted to reduce patient harm and improve care coordination. This network of hospitals will be supported by integrated quality and process improvement advisors, practice transformation resources, educational content, and a proven data management and technical assistance program.

Compass HIIN work is underway, with the development of new educational events, resources, and the update to the Compass HIIN Reporting Database. Improvement Advisors will soon be reaching out to hospitals to schedule individual site visits once again. HIIN CEO Charters have been sent our to all Iowa hospitals, as well as hospitals in South Dakota, Illinois and Colorado. Please work with your hospital CEO to complete and return the charter to secure your participation in the Compass HIIN. Iowa Healthcare Collaborative selected to continue improvements in patient safety
Hospital Improvement Innovation Networks to continue patient safety improvement efforts started under the Partnership for Patients initiative
 
Des Moines, Iowa – Iowa Healthcare Collaborative (IHC) has been selected as one of 16 national, regional, or state hospital associations, Quality Improvement Organizations, and health system organizations to continue efforts in reducing preventable hospital-acquired conditions and readmissions. The Hospital Improvement Innovation Network contracts awarded build upon the collective momentum of the Hospital Engagement Networks and Quality Improvement Organizations to reduce patient harm and readmissions. This announcement is part of a broader effort to transform our health care system into one that works better for the American people and for the Medicare program.
 
“IHC and our partners, are thrilled to be selected as a Hospital Improvement Innovation Network. We will operate as the Compass Hospital Improvement Innovation Network across four states including Iowa, Colorado, Illinois and South Dakota, assisting hospitals and their communities, as they build upon the success of the first Hospital Engagement Network programs,” said Tom Evans, MD President/CEO of Iowa Healthcare Collaborative. “The Partnership for Patients initiative provides resources to help hospitals provide better care, engage patients, and improve community healthcare outcomes. We are very proud of Iowa hospitals and their work to improve patient care by reducing patient harm. Iowa hospitals combined efforts have resulted in over 6,000 adverse events avoided and approximately $60 million in reduced healthcare costs.”
 
Through 2019, these Hospital Improvement Innovation Networks will work to achieve a 20 percent decrease in overall patient harm and a 12 percent reduction in 30-day hospital readmissions as a population-based measure (readmissions per 1,000 people) from the 2014 baseline. Efforts to address health equity for Medicare beneficiaries will be central to the Hospital Improvement Innovation Networks efforts. CMS will monitor and evaluate the activities of the Hospital Improvement Innovation Networks to ensure that they are generating results and improving patient safety.
 
“We have made significant progress in keeping patients safe – an estimated 2.1 million fewer patients harmed, 87,000 lives saved, and nearly $20 billion in cost-savings from 2010 to 2014 – and we are focused on accelerating improvement efforts,” said Patrick Conway, M.D., CMS acting principal deputy administrator and chief medical officer. “The work of the Hospital Improvement Innovation Networks will allow us to continue to improve health care safety across the nation and reduce readmissions at a national scale – keeping people as safe and healthy as possible.”

The Partnership for Patients model is one of the first models established in 2011 to be tested under the authority of section 1115A of the Social Security Act (the Act) with the goal of reducing program expenditures while preserving or enhancing the quality of care. Since the launch of the Partnership for Patients and the work of Hospital Engagement Networks in collaboration with many other stakeholders, the vast majority of U.S. hospitals have delivered results as demonstrated by the achievement of unprecedented national reductions in harm. CMS believes that the upcoming work of the Hospital Improvement Innovation Networks, working as part of the Quality Improvement Organization’s work to improve patient safety and the quality of care in the Medicare program, will continue the great strides made in improving care provided to beneficiaries.

IHC Patient Safety Award - Nominations Now Being Accepted
The Iowa Healthcare Collaborative (IHC) is currently accepting nominations for the IHC Patient Safety Award for healthcare providers and healthcare organizations who have demonstrated outstanding leadership and achievement in patient safety. The Patient Safety Award aims to raise the standard of healthcare in Iowa by promoting a culture of continuous improvement in quality, safety and value.
 
This award honors patient safety champions that support the following initiatives:
  • Improve patient safety
  • Reduce the risk of harm
  • Keep patients at the center of care
Eligibility
All practicing healthcare providers (individuals, organizations, healthcare teams, physician practices, etc.) who care for Iowa patients may submit a nomination for the IHC Patient Safety Award.
 
Nomination Deadline
We encourage you to share this opportunity with your organization or submit a nomination for someone you think should be recognized.  The nomination deadline is December 1, 2016.  See the brochure for more information on how to register.
 
Categories
Nominations will be accepted for individuals and/or organizations that demonstrate achievement in patient safety in the following categories:
  • Achievement in reducing healthcare-associated infections
  • Achievement in improving care transitions (i.e. readmissions, community care coordination)
  • Achievement in reducing preventable conditions (e.g. hospital-acquired, provider preventable, etc.)
  • Achievement in improving culture of safety/safety across the board (i.e. becoming a high-reliability organization)
  • Achievement in improving person and family engagement
 Award Presentations
The award will be presented to the winning organization, department, or individual at the spring 2017 IHC Patient Safety Conference on March 14, 2017. The finalists will also be publicly recognized.

Learning Community

The next Iowa State Innovation Model (SIM) Learning Community event will be held on November 9th at the Meadows Events and Conference Center in Altoona. The day will be an exciting and unique opportunity to hear from healthcare leaders throughout the state, gain national perspectives and learn the next steps of the SIM initiative in Iowa.

Database Update:
CEO Dashboards


IHC recently released a new report that hospitals can populate from the PfP HEN Database. The CEO dashboard is the newest enhancement to the database and gives a “stoplight” view of current hospital progress toward HEN goals. Included below is the legend which notes the degree of hospital performance related to HEN 2.0 goals identified within the populated report (baseline compared to timeframe specified). Goals for the HEN 2.0 include a 40% improvement in each healthcare acquired condition (adverse drug events, falls, pressure ulcers, healthcare associated infection, etc), as well as a 20% improvement in readmissions.
 
Hospitals have the ability to customize time frame and populate a report, for a limited selection of HEN metrics, that includes Hospital rate and percent improvement, as well as Peer and State Averages. This long awaited report is now available on the PfP HEN Database.

While PfP HEN database users can auto populate dashboards on demand, IHC HEN staff will also send HEN updates and CEO dashboards to hospital CEOs bi-annually.
 
CEO dashboards can be found on the reporting page along with other HEN reports.
