Electronic Form W-2
The St. Vrain Valley School District is required by the IRS to furnish all employees a Form W-2 Wage and Tax Statement each calendar year to be used in completion of the employees’ annual tax returns. Employees may elect to receive their Form W-2 electronically online through the iVisions Employee Self Service Web Portal.

 The Benefits Open Enrollment is now open. Eligible employees may enroll for their 2017 benefits through Friday, November 18. For employees who do not wish to make any enrollment changes to their medical, dental, vision, supplemental life or short-term disability or don’t wish to change their pre- or post-tax deduction status, you DO NOT need to complete an enrollment. Your current benefit elections will roll over for 2017. EXCEPTION: If you wish to enroll in a Health Care or Dependent Care Flexible Spending Account, you must enroll by the enrollment deadline. All changes will be effective January 1, 2017 and no late enrollments can be accepted!

 Protect Yourself from the Flu!
If you are a regular status district employee and were unable to attend last month’s onsite flu shot clinics, it’s not too late to get a district-paid flu shot. Eligible employees can still get a district-paid flu shot by contacting Delene Gunderson in the benefits office (ext. 57383) to request a voucher. Take the voucher to any Safeway pharmacy before November 30 to receive your free flu shot and a 10% grocery discount coupon.
As a reminder, both the CNIC health plan and the Kaiser Permanente health plan cover flu shots at 100% under the preventive health care benefit. Individuals covered on the CNIC health plan can get their flu shot either through their doctor’s office or at in-network pharmacies. Kaiser facilities are offering walk-in flu shot clinics at most Kaiser medical offices from 8 a.m. – 6 p.m. through November 30. Shots can also be administered through Kaiser providers during scheduled office visits.

 Registrations Open for Onsite Biometric Health Screenings -Do Something Good for Yourself and Get Rewarded!
We hope you’ll join us for a complimentary, onsite biometric health screening this December. In addition to getting a better picture of your current health and potential health risks, you can earn a $50 gift card to your favorite grocery store as a reward simply for participating in this voluntary screening. In addition to the $50 grocery gift card, five lucky employees will win a $200 Master Card gift card to help with their holiday shopping! Screenings are district-paid and confidential.
The district is partnering with a new wellness partner—Interactive Health—to conduct this year’s onsite health screenings. To qualify for the full $50 incentive, eligible employees must complete an online Health Risk Assessment (available on the Interactive Health registration website) and participate in the onsite biometric health screening in December. See next page for additional information on how to score your $50 gift card and qualify for the $200 gift card drawing!
