2017 IHC Patient Safety Award Reminder

IHC is accepting nominations for the 2017 Patient Safety Award! The Compass HIIN team encourages all Iowa hospitals to participate by taking the time to recognize ALL hospital team members and their hard work. Your hospital narrative can be simple, and we would be more than happy to assist you in drafting your narrative!

IHC Patient Safety Award - Nominations Now Being Accepted
The Iowa Healthcare Collaborative (IHC) is currently accepting nominations for the IHC Patient Safety Award for healthcare providers and healthcare organizations who have demonstrated outstanding leadership and achievement in patient safety. The Patient Safety Award aims to raise the standard of healthcare in Iowa by promoting a culture of continuous improvement in quality, safety, and value.
 
This award honors patient safety champions that support the following initiatives:

  • Improve patient safety
  • Reduce the risk of harm
  • Keep patients at the center of care
We encourage you to share this opportunity with your organization or submit a nomination for someone you think should be recognized.  The nomination deadline is December 1, 2016. Visit www.ihconline.org for more information.  Please download the brochure and email or fax it to IHC. 
INR Greater Than 5 Quality Improvement Project
Your HIIN Team will begin the INR greater than 5 Project on November 29th. This project will include 3 webinars provided by our faculty Pharmacist Dr. Brian Isetts. We will be giving valuable resources and assisting in Quality Improvement in this area. The webinars will be 30 minutes followed by coaching for a limited amount of hospitals.

The advantages to participating:
  • During this project we would like to provide you with evidence based best practice for managing patients on warfarin.
  • Individual and group coaching with Dr. Brian Isetts, PhD
  • Opportunity to be part of a Pharmacist lead quality improvement project
We ask the following commitments to participate:
  • Participate in 3 Webinars, November 29th, January 19th and February 23rd all at 11:00 AM
  • Do process improvement as recommended during webinars during the weeks between webinars
  • Monitor INR greater than 5 in your inpatient population and report this to the HIIN Database
  • Monitor and record INR greater than 5 that come into your ED
  • Have a team based work group participate in the webinar and improvement effort including your Pharmacist, Quality Lead, Bedside Best Practice Leader and Champion
  • Have one team member or more participate in IHC Patient Safety Conference on March 14th
During this project we would like to provide you with evidence based best practice for managing patients on warfarin. The project will start with a webinar on November 29th at 11:00 AM (45 min. – 1 hour), please be ready to share successes and challenges with managing patients on warfarin therapy.

Compass HIIN Virtual Learning Community

When: December 1st, 2016 starting at 8:30am

What: The intent of the Iowa Healthcare Collaborative HIIN Learning Communities is to assist network hospitals in carrying out the national Partnership for Patients goals of reducing avoidable hospital readmissions and decreasing preventable hospital-acquired conditions. Through these Learning Communities, IHC will provide assistance in improvement initiatives and the spread of best practices across a high-quality performing network.

Who: Physicians, nurse managers, critical care nurses, staff nurses, pharmacists, infection preventionists, risk managers, quality professionals, case managers, long-term care, and home healthcare providers.

The Work Plan is intended to capture current work as it is implemented in your hospital.  The Compass HIIN project is about deploying best practices and spreading the successes to drive improvement. The Work Plan will be completed on the HIIN Reporting Database once it is made available, but for now, you can start my filling out the preparation template below:
Compass HIIN Toolkit Now Available!
The Compass HIIN Toolkit was finalized earlier this month and sent out to hospitals. This toolkit provides instructions on data reporting, data sources, reporting deadlines, measure instructions, how to run reports, measure resources, tips and tricks, and more!
