Celebrating Four Years of
Prepare for Your Care

Happy New Year, and Happy Birthday to Prepare for Your Care!  We first launched PREPARE as an interactive, web-based ACP planning tool in January 2013.  Since then, more than 73,000 people have visited the site, with over 1.6 million page views. In the past four years, we have made many updates and improvements, with more to come:

  • We tested the site with culturally diverse populations and made modifications to ensure that it meets their needs. 
  • We developed a Spanish version of PREPARE, thanks to the California Healthcare Foundation, and are seeking funding to complete a Chinese version later in 2017. 
  • The site now includes optional text voice-over and closed captioning, making it accessible to those with visual or hearing impairment.
  • With support from the Gordon and Betty Moore Foundation, we will soon update PREPARE to include customizable, easy-to-read advance directives for all 50 states, in both Spanish and English.
Funding Update:  Gordon and Betty Moore Foundation and the Stupski Foundation
As part of the grant awarded in 2016, we have consolidated feedback about PREPARE from the public, patients, and topical experts, and have worked with two marketing firms to streamline and update the website. These updates will promote broader engagement with PREPARE in both healthcare and  community settings. We also continued developing easy-to-read English and Spanish advance directives, as noted above. 

Also in 2016, the Stupski Foundation awarded $3 million to the UCSF Palliative Care Program to create a scalable, comprehensive, and coordinated palliative care system for all UCSF Health patients. As part of the grant, PREPARE will be used in outpatient primary care clinics and inpatient settings to support advance care planning. 

Raising Awareness of ACP and PREPARE
I was honored to speak at and participate in several conferences in the second half of 2016.  All provided great opportunities to share the PREPARE story.

  • American Society of Clinical Oncology 2016 Palliative Care in Oncology Symposium
  • Commonwealth Club session on End-of-Life Challenges and Solutions
  • The 2016 World Cancer Congress
  • The 2016 PCORI Annual Meeting
  • The John A. Hartford Foundation Palliative Care Learning Lab
  • The National Academies of Sciences, Engineering and Medicine Roundtable on Quality of Care for People with Serious Illness.
Looking ahead to 2017, we will publish the results of the VA randomized trial, and will be completing our R01 & PCORI-funded trials. Early findings reinforce the role of PREPARE as a valuable tool for engaging older adults on ACP and completing ACP documents.
