Health and Liability Forms
There have been a few changes this year with the Health and Liability forms. There is no longer a supplemental Code of Conduct form that is needed. The H&L Forms include acknowledgment that you and your students have reviewed and agreed to the posted COC online. This is the same for our Youth Safety Guideline section for every adult.
Meals
Friday Lunch is now included in the registration fee. We also need one (1) Veggie/Allergy Meal Request Form per troupe for all of your veggie and allergy meal requests. Festival Meals are Nut Free, and allergy requests can be made for Soy, Dairy, Gluten, and Shell Fish. All allergy meals can be vegetarian as well. This is required. Designation on the Health and Liability form is for record only and will not be used by Jay's Catering.
Recognizing Achievement
