Governing Board -- July 9, 2018

  • Board moved to accept minutes from June, approved.
  • Board moved to accept updated financial statement, approved.
  • Board moved to accept the following nominations and updates to the Governing Board and Executive Committee, approved:
    • Rosalinda Kibby to fill Public Hospital Seat 1 (replacing Theresa Sullivan), approved.
    • Kyle Kellum to fill Grant County CHI Seat (replacing Rosalinda Kibby), approved.
  • CHI Updates: The 3 Coalitions for Health Improvement are currently conducting a community feedback survey to learn more about opportunities and barriers to health across the region. Take survey here.
  • Board reviewed the motion to approve a contract with Providence Center for Outcomes Research and Education (CORE) for up to $103,383 through June 2018 to support continued technical assistance and consultation around data needs, and to expand the scope of data analytic support: After review, the Board decided to defer the motion for further discussion at the next Governing Board Retreat.
  • Board motion to approve a contract with the Center for Community Health and Evaluation (CCHE) for up to $7,000 to develop and support an evaluation plan for NCACH's Transformation projects: approved.
  • Board motion to approve the Whole Person Care Collaborative (WPCC) Change Plan evaluation and Stage 2 Funding models for WPCC Learning Community members: approved with amendments.
  • Board motion to approve NCACH entering a contract with the University of Washington AIMS Center for consulting services in the amount of $48,000: approved.
  • Transitional Care and Diversion Interventions (TCDI) Workgroup update: The TCDI Workgroup has developed an application for partnering providers to select approaches and receive funding to implement transitional care management and Emergency Department (ED) Diversions strategies. The TCDI Workgroup will be finalizing partner applications and a funding model to bring as a motion to the Board in August 2018.
    • North Central Emergency Care Council (NCECC) provided an update on the Phase 1 of the Community Paramedicine plan approved by the Board in May 2018. In partnership with the Transitional Care and Diversion Interventions Workgroup, NCECC is proposing to partner with the NCACH to perform a state assessment and incorporate tactics to reduce EMS transport to the ER and reduce readmission rates for chronic disease and high-risk patients through a community paramedicine model that will also standardize how data is collected and shared across EMS providers in the North Central region.
  • Board motion to amend the funding period for the Pathways Community HUB design period from June - December 2018 to June - September 2018: approved.

Whole Person Care Collaborative -- July 9, 2018

  • WPCC approved minutes from June's meeting
  • Discussion: Site visit recaps
    • NCACH staff completed 17 one-on-one site visits with WPCC Learning Community members to discuss change plans and check in about the LAN process over the month of June
  • WPCC Workgroup updates: WPCC Workgroup met on July 2, 2018
    • WPCC was asked to give feedback on the Change Plan evaluation criteria and Stage 2 Funding framework developed by the Workgroup
  • Future LAN Offerings: The WPCC reviewed future LAN offerings for WPCC Learning Community members, which include 2 upcoming workshops hosted in Wenatchee
  • Next meeting: September 10, 2018

Whole Person Care Collaborative Workgroup -- July 2, 2018

  • Bulk of the meeting was spent reviewing proposed change plan evaluation criteria and stage 2 funding framework.
  • Input from the Workgroup shaped revisions in time for recommendations that were endorsed at the July 9th WPCC and Governing Board meetings.

HUB Advisory Board -- July 12, 2018

The HUB Advisory Board, convened by Community Choice (dba Action Health Partners), meets bi-weekly. The group serves as an advisory body to support the HUB as it enters operation in Fall 2018. The Pathways Community HUB Workgroup is no longer meeting. You can keep up with all things HUB by visiting our Care Coordination page on the NCACH website, where we'll share updates and meeting documents.

  • HUB Training and Updates:
    • HUB Care Coordination Agencies are currently being established in Moses Lake
    • HUB software platform being established
  • HUB Outreach currently underway
    • Still identifying places to do outreach in Moses Lake area
  • Next meeting: August 9, 2018


Opioid Project -- July 20, 2018

  • Workgroup reviewed 11 funded Rapid Cycle Opioid Project applications and reporting requirements due by award recipients
  • Workgroup reviewed overlap with other NCACH Medicaid Transformation Project objectives including the Whole Person Care Collaborative Learning Community's Change Plans, Transitional Care and Diversion Intervention Strategies, and updates on the Pathways Community HUB.
  • Workgroup reviewed available funding for the Opioid Project for the remainder of the Medicaid Transformation and determined how to best use the available funding. Ideas include hosting conferences as educational sessions and releasing more funding through Rapid-Cycle applications in the following years.

Transitional Care and Diversion Interventions -- July meeting held remotely

  • In July, both Emergency Department and Transitional Care Management partners reviewed their sections of the TCDI hospital application and provided input that lead to the final draft. 
  • The TCDI workgroup cancelled the workgroup meeting in July.  In lieu of the workgroup meeting, TCDI members voted electronically on the application process and the recommendations they wanted to present to the Governing Board.  The final recommendation approved via electronic vote by the TCDI workgroup is as follows:
    • The TCDI workgroup recommends the NCACH Governing Board approve the process (outlined in the TCDI Hospital application) of engaging hospital partners in project 2C and 2D and funding not to exceed $759,000 to support partnering providers.

Coalitions for Health Improvement (CHIs)

  • Chelan Douglas CHI met July 11, 2018
    • Members focused on developing strategies to source transportation solutions for job seekers who may be in treatment or lack transportation to and from job interviews. August's meeting will focus on more solution-building as well as a presentation on the Pathways Community HUB.
    • Next meeting: August 8, 2018
  • Grant County CHI 
    • From June's meeting: Members identified county-level priorities through facilitated discussions, and decided that the Grant County CHI would like to focus on working with communities affected by substance use disorder. Grant County CHI Board Seat, Rosalinda Kibby, assumed the Public Hospital District Governing Board Seat and Kyle Kellum, Samaritan Healthcare, was nominated to serve on the NCACH Governing Board as the Grant County CHI Board Seat.
    • Next meeting: August 21, 2018
  • Okanogan County CHI
    • From June's meeting: Members heard from Okanogan County-based Rapid-Cycle Opioid Project Award recipients (Family Health Centers and Washington-211) and identified ways for the CHI to directly support local initiatives. Washington-211 also gave a presentation to the CHI highlighting the services provided and available for the community.
