Cultural Competency and Health Literacy Master Training (TTT) Program
Tuesday, November 13 @ 8:30 a.m. - 5:00 p.m.
The Communities we serve are growing in cultural and linguistic diversity. There is a need to inspire cultural change within organizations. Join the SCC in the next Cultural Competency and Health Literacy Master Training (TTT) Program, an interactive program in which participants will have a hands-on learning experience to build better communities. 
 
Post-training, Master Trainers will receive a certificate of completion and the ability to host training sessions throughout the region. Master Trainers will be able to facilitate two different trainings: (1) 2 hour CCHL training sessions for workforce professionals and (2) 7.5 hour master training sessions for future master-trainers.
 
Participants must apply online by end of business Friday, October 26th, 2018. This is a required application process, please note completing this application does not confirm attendance to this program. Upon completing this application, you will receive follow up from a CCHL lead with next steps.
 
Due to the interactive nature of this program, registration is limited to 30 participants. A maximum of 2 participants per organization will be selected to attend. Preference will be given to those applicants with previous facilitation experience and those who work regularly with under-served populations within their professional roles.

"Said No Teen Ever" - New Public Service Announcement

One in five young people lives with a mental health or substance use challenge, yet few reach out for support.  As youth across America head back to school this season, Mental Health First Aid USA has released a new Public Service Announcement (PSA) - Said No Teen Ever - to underscore the value of Youth Mental Health First Aid. Watch four individual PSAs to highlight mental health challenges that young people experience every day like anxiety, depression, eating disorders and self-harm.

Healthy Sundays

Healthy Sundays, exemplifying CHS's mission to care for those in need, is one of the system's most prolific outreach efforts. Launched in 2005, the Healthy Sundays network provides preventive health screenings and education to members of medically underserved communities on Long Island. Partnering with churches and community centers across Nassau and Suffolk, the initiative continues to grow, offering the expertise of hundreds of CHS volunteers.

Healthy Sundays address prevalent medical conditions—such as obesity, diabetes, and hypertension— to improve quality of life, decreasing morbidity and mortality. Free blood pressure and body mass index screenings, as well as flu shots, are administered throughout the year. Educational materials are provided in Spanish and English, and representatives from community benefit organizations Fidelis Care and United Healthcare, among others, also attend these events.

Breast Cancer Support Groups
The Adelphi NY Statewide Breast Cancer Hotline and Support Program offers ongoing, free support groups facilitated by a social worker or master level social work intern. Unless otherwise noted, all groups meet at the Adelphi School of Social Work, Adelphi University, 1 South Ave., Garden City, NY 11530

Women who are recently diagnosed with breast cancer will have the opportunity to meet with others in similar situations in this group.

Taking care of a person with breast cancer can be stressful. This group is designed to address the needs of caretakers. Any person who is experiencing the stress and anxiety of caring for a loved one with breast cancer is welcome.

Time: 6:00 - 7:30 PM 
Women under the age of 40 who are diagnosed with breast cancer have unique needs. This support group will focus on those unique needs while allowing the members to share their feelings in a safe, supportive environment.

Meet with other women who have been diagnosed with Stage 4/metastatic breast cancer.

Monthly Groups

This group is for any breast cancer survivor who has received their last treatment more than 18 months ago. The group will meet monthly to discuss a book that focuses on cancer and coping. Discussing a book with characters on similar journeys can elicit empathetic conversations about our own journeys and connect us to other survivors.

Epilepsy Community Education and Outreach Programs
EPIC Long Island believes that with education comes understanding and acceptance. As part of the ongoing Community Education and Outreach programs, the Foundation seeks to communicate with all persons who have seizure disorders. The Foundation also strives to educate teachers, health care professionals, corporations, and industry, as well as the general public, about the causes, treatment, and consequences of epilepsy and the proper first aid response to seizures. Wyandanch Baby Cafe

The Baby Cafe is a free drop-in session offering information and support for pregnant and breastfeeding moms. A trained breastfeeding counselor is available at every session. Mothers can expect a relaxed, informal, and safe environment in which they can enjoy the company of other breastfeeding mothers and receive breastfeeding information and assistance from a trained counselor. Babies, young children and mom’s support person are welcome.
 
The Wyandanch Baby Cafe is held twice a month on select Saturdays from 11:30 a.m. to 1:30 p.m. in the lower level of the Wyandanch Community Nazarene Church, 58 Cumberbach Street, Wyandanch, NY 11798.

Behavioral Health Strategies for Post-Acute Care, Physical Assessment and Sepsis
Monday, November 19 @ 8:00 a.m. - 4:30 p.m.

This session will provide long-term care leaders the elements required for Behavioral Health under the CMS’s new Requirements of Participation (RoP) for Skilled Nursing Facilities. Attendees will also be provided with the key components of a comprehensive physical assessment process for the post-acute care resident.
 
Behavioral Health Strategies for Post-Acute Care – 2 hours 
This session will provide LTC leaders the elements required for Behavioral Health under the NEW Requirements of Participation for Skilled Nursing Facilities.

Upon completion of the class the attendees should be able to:

  • Identify the required elements for behavioral health for Phase 2 of the NEW RoP
  • Learn where to find helpful resources to assist with the development of a behavioral health program
  • Describe three leadership strategies for caring for someone with behavioral or mental health issues

Physical Assessment and Sepsis – 5 hours 
This engaging presentation will provide attendees with the key components of a comprehensive physical assessment for post-acute care residents. This program will include a presentation along with a demonstration by the presenter. This presentation will provide attendees with key strategies for using a resident’s history, guiding the assessment process, decision-making regarding notifications, and making recommendations and quality care planning decisions. Additionally, attendees will be provided with recommendations on the identification and management of residents with Sepsis.

Upon completion of this program, attendees will be able to:  

  • Collect and review key resident history and current health condition information
  • Identify the skills necessary in the resident/resident representative interview process
  •  Verbalize steps for a head-to-toe assessment, including techniques such as inspection, palpation, percussion, and auscultation
  • Describe the process for documentation and notification for normal and abnormal findings
  • Verbalize how to utilize the information from the physical assessment to develop and/or update the resident’s individualized plan of care
  • Provide the definition of sepsis and discuss recommended guidance on the identification of possible sepsis and next steps for quality management


Target Audience: Skilled Nursing Facilities

A light breakfast and lunch will be provided. There are three training dates to choose from:

Beginner and Intermediate Courses in Excel
Tuesday, October 23 & Tuesday, November 6 @ 9:00 a.m. - 12 p.m.

These workshops provide an understanding of the popular spreadsheet package. Each course consists of two classes. Topics for the beginner course include: creating, editing and formatting a spreadsheet; utilizing basic formulas and functions, and charting. The Intermediate workshop builds upon basic skills of the first course and includes understanding formulas, functions, range names, and chart making. If you take the Introductory Course, you can then take the Intermediate Course immediately following. We planned the classes two weeks' apart so you can practice in between classes.

Target Audience: Staff and Managers who need a better understanding of Excel to assess data

Introduction to Emotionally Focused Therapy

Emotionally Focused Therapy for Couples (EFT) presented by Dr. Mayer Bellehsen and Aviva Lauren Rizel, MA LMFT is an evidence-based treatment for working with couples in distress that is highly adaptable to different contexts. This course will provide an introductory overview of EFT. The stages of EFT will be reviewed along with experiential opportunities to practice basic techniques 3.5 continuing education hours available for LMSW/LCSW/LMHC/LCAT/LMFT.

Quality Assurance & Performance Improvement (QAPI) for Skilled Nursing Facilities
Friday, October 26 @ 8:30 a.m. - 4:30 p.m.

The Nassau-Queens PPS invites you to participate in a one-day training session on Quality and Performance Improvement in Skilled Nursing Facilities (SNF). Quality improvement (QI) and performance improvement (PI) concepts can be incorporated by staff in any role to improve workflows and patient outcomes. 

The training will be rooted in the Institute for Healthcare Improvement (IHI) Model for Improvement and will be contextualized for the SNF environment. Participants will identify opportunities for improvement in their daily lives, be introduced to the Model for Improvement, learn how to perform root/cause analyses, and create improvement plans using examples from their practices.

The training will be interactive and will include lecture/didactics along with learning activities. The session will be facilitated by Regina Neal of Qualis Health.

Who Should Attend the Training: The training is open to all staff who work in the SNF setting, and is recommended for nurses, nurse assistants, social workers, and quality assurance staff. GNYHA recommends that multiple team members from a facility. A diverse representation of roles maximizes the extent to which the team learns to jointly approach challenges related to workflows and overall PI. Managers, administrators, and nursing and physician leaders are also encouraged to attend with their teams.

Motivational Interviewing

Back by Popular Demand! The Nassau-Queens PPS invites you to participate in a one-day training session on Motivational Interviewing (MI), an engagement technique that can be used by health care and social service professionals to support patients and clients in self-management and assist with health and social needs. The training is sponsored by the Greater New York Hospital Association (GNYHA) and is funded by the New York State Department of Health’s (DOH) Health Workforce Retraining Initiative (HWRI). A light breakfast and lunch will be provided.

MI is a collaborative, patient-centered technique to elicit and strengthen motivation for change. MI can be used as a mechanism to help individuals identify priorities and engage in treatment and self-management activities. MI techniques provide a framework to guide discussions with patients and clients, and prompt patients to participate in decisions that affect their health.

Target Audience: Training is open to, but not limited to Community Health Workers, Health Coaches, care transition staff, nurses, social workers, care and case managers, hospital staff and community-based practice staff.

NYSHealth's annual conference on Empowering Health Care Consumers will take place Monday, November 5, 2018.

The keynote speaker will be Dr. Eric Manheimer, former medical director at Bellevue Hospital. He is currently a producer, a writer, and the medical consultant for the NBC TV series New Amsterdam, based on his book 12 Patients: Life and Death at Bellevue Hospital and his own experience as a physician turned cancer patient.

The "Partnership in Action: Patients and Providers" panel features patients and providers discussing how they are working together to advance patient-centered care, meaningful engagement, and partnerships.

The "How Listening to Patients Leads to Innovation" panel features examples of how providers, payers, and other sectors are elevating patient voices, with resulting positive outcomes.

  • Train and support all teaching, administrative and counseling staff in every phase of the Calm Classroom program

PREPaRE Workshop 2 Crisis Intervention and Recovery: The Roles of School-Based Mental Health Professionals
Monday, November 19 & Tuesday, November 20 @ 7:30 a.m. - 4:00 p.m.
This 2-day workshop provides school-based mental health professionals and other school crisis intervention team members with the knowledge necessary to meet the mental health needs of students and staff following a school-associated crisis event. With updated research and crisis intervention strategies, this workshop teaches participants how to prevent and prepare for psychological trauma, helps to reaffirm both the physical health of members of the school community and students’ perceptions that they are safe and secure, evaluates the degree of psychological trauma, responds to the psychological needs of members of the school community and examines the effectiveness of school crisis intervention and recovery efforts.

A Healing and Empowerment Tool-Guided Memoir Writing- brought to you by Herstory©
Tuesday, November 27 & Thursday, November 29 @ 8:30 a.m. - 2:30 p.m.
This two-day workshop will provide an introduction to an innovative way of using memoir to help young people to know and cherish those stronger, more lovable parts of themselves that they have buried or left far behind, while daring those around them to care. It will assist counselors, mental health professionals, teachers and other school staff to strengthen culturally responsive instruction and foster an environment of empathy, inclusion, empowerment, and healing in the classroom, school building and beyond. 

Herstory© Writers Workshop’s tools for guided memoir writing is an adjunct to other interventions within the recovery arena as well as a tool to meet ELA Standards. Herstory brings unheard voices into the public arena, transforming lived experiences into written memoir powerful enough to change hearts, minds, and policy.

Youth Mental Health First Aid
Thursday, December 6 @ 8:00 a.m. - 2:30 p.m.
This program teaches school personnel, family members, social services and agency staff how to recognize and help a youth who may be developing the signs and a mental health disorders/crises in youth, or experiencing a mental health or substance abuse crisis. The 5-step Mental Health First Aid action plan is explored and applied to youth mental health disorders and crisis situations through the use of discussions, case examples and videos. This action plan includes approach skills, effective communication of concern and support, and connecting youth and families to professional and community resources.

11th Annual Suicide Prevention Coalition of Long Island Symposium
Tuesday, December 11 @ 8:00 a.m. - 3:30 p.m.
Dr. Christine Moutier, Chief Medical Officer of the American Foundation for Suicide Prevention, will help us make sense of the current body of data regarding risk of suicide among our youth and young adults, the rise in the rate of teen and young adult death due to suicide, and specifically, how we can use what is known about the impact of social media in our work to identify, address and reduce risk.  What are the conversations that we should be having with youth and young adults and how do we frame those conversations?  In addition, Dr. Moutier will discuss cyberbullying and risk of suicide. 5 continuing education hours available for LMSW/LCSW/LMHC/LCAT/LMFT.

Learning 2 Breathe Intensive Training
Monday, February 4 & Tuesday, February 5 @ 8:30 a.m. - 3:00 p.m.
This two-day workshop is designed to help you facilitate the growth of mindfulness in adolescents using the Learning to BREATHE (L2B) program. Demonstration, small-group practice and group feedback are done for each of the six themes of the program. Discussion about the role of the teacher and the pedagogical elements of mindfulness-based programs is part of the workshop. We will be engaged in a mix of demonstration, practice teaching and group discussion throughout both days. The workshop is intended to prepare participants to actually teach the program, so it relies more on practice than on presentation of theoretical and research background information. A short overview of the curriculum will be shared on day 1. All materials for the workshop will be provided. This training can be used to support the implementation of the amended New York State Education Law in Mental Health Education Literacy for schools.

Job Title: Internship Opportunity in Environmental Health

Summary:
As over 1.3 million residents of Nassau County rely on groundwater to meet their drinking water needs, the County is committed to protecting our groundwater from contamination. The proper closure of underground heating oil tanks can eliminate the threat of petroleum contamination of the aquifer system from leaking tanks. The Department’s Office of Toxic and Hazardous Materials Storage administers a Small Heating Oil Tank Abandonment Program which is the recipient of a Model Practice Award from the National Association of City & County Health Officers. We are seeking a college intern to assist in three different phases of the program. Click here for internship and department objectives.

Qualification:
  • 8-10 hrs. per week for the Fall or Spring semester or a minimum of 200 hours for a summer internship
  • Preference for a college Junior/Senior or Masters Degree candidate interested in a future career in the Environmental or Public Health field.
  • A basic knowledge of the geology/hydrology of Long Island
  •  Knowledge of Microsoft Access, Excel, Word and PowerPoint
  • Good communication skills (verbal and written)
