The Iowa State Innovation Model (SIM) Learning Community will be held on November 6 at the IHA Conference Center in Des Moines. The day will be an exciting and unique opportunity to hear from healthcare leaders and gain national perspectives.
  
Sessions for the day will include:
•    Defining Community Partnership Integration to Drive Healthcare Transformation
•    Iowa Rural Update: Building Economic Capacity and Healthcare Excellence in Rural Iowa
•    Achieving Value from Community Health Initiatives
•    Current Trends in Payment Design 
  
The SIM Learning Community will bring together healthcare professionals from across the care continuum to provide Iowa communities the tools and necessary depth of understanding needed to improve outcomes, increase patient safety and decrease the cost of care for all Iowans. It will underscore the cooperative nature of the project across the healthcare stakeholder communities of provider, payer and public health and statewide strategies promoted in the SIM project. 
  
Attendance is FREE and open to all interested stakeholders.
