I, the undersigned, hereby acknowledge that I am a member in good standing with 100 Women Who Care South Georgian Triangle (100 Women SGT).
By completing this Proxy Ballot, I understand that I am giving authority to the “Designee” named below to vote for me, in my absence, at the upcoming 100 Women SGT meeting. I acknowledge that this Designee has authority to submit my vote by naming a charity as they so choose, having heard the various charity presentations at the meeting. I also confirm that the Designee is a member in good standing with 100 Women SGT.
I understand that this Proxy Ballot must be submitted to a member of the 100 Women SGT Steering Committee either electronically by completing the sections below and pressing, "subscribe", or printed and submitted in person prior to the start of the meeting. I further understand that PRIOR to the start of the meeting, the Designee is to make herself known to a member of the Steering Committee at the meeting registration table for the purpose of receiving my ballot.
This Proxy Ballot is applicable only to the meeting date noted below.
