Behavioral Health Strategies for Post-Acute Care, Physical Assessment and Sepsis
Monday, November 19 @ 8:00 a.m. - 4:30 p.m.

This session will provide long-term care leaders the elements required for Behavioral Health under the CMS’s new Requirements of Participation (RoP) for Skilled Nursing Facilities. Attendees will also be provided with the key components of a comprehensive physical assessment process for the post-acute care resident.
 
Behavioral Health Strategies for Post-Acute Care – 2 hours 
This session will provide LTC leaders the elements required for Behavioral Health under the NEW Requirements of Participation for Skilled Nursing Facilities.

Upon completion of the class the attendees should be able to:

  • Identify the required elements for behavioral health for Phase 2 of the NEW RoP
  • Learn where to find helpful resources to assist with the development of a behavioral health program
  • Describe three leadership strategies for caring for someone with behavioral or mental health issues

Physical Assessment and Sepsis – 5 hours 
This engaging presentation will provide attendees with the key components of a comprehensive physical assessment for post-acute care residents. This program will include a presentation along with a demonstration by the presenter. This presentation will provide attendees with key strategies for using a resident’s history, guiding the assessment process, decision-making regarding notifications, and making recommendations and quality care planning decisions. Additionally, attendees will be provided with recommendations on the identification and management of residents with Sepsis.

Upon completion of this program, attendees will be able to:  

  • Collect and review key resident history and current health condition information
  • Identify the skills necessary in the resident/resident representative interview process
  •  Verbalize steps for a head-to-toe assessment, including techniques such as inspection, palpation, percussion, and auscultation
  • Describe the process for documentation and notification for normal and abnormal findings
  • Verbalize how to utilize the information from the physical assessment to develop and/or update the resident’s individualized plan of care
  • Provide the definition of sepsis and discuss recommended guidance on the identification of possible sepsis and next steps for quality management


Target Audience: Skilled Nursing Facilities

A light breakfast and lunch will be provided. There are three training dates to choose from:

PREPaRE Workshop 2 Crisis Intervention and Recovery: The Roles of School-Based Mental Health Professionals
Monday, November 19 & Tuesday, November 20 @ 7:30 a.m. - 4:00 p.m.
This 2-day workshop provides school-based mental health professionals and other school crisis intervention team members with the knowledge necessary to meet the mental health needs of students and staff following a school-associated crisis event. With updated research and crisis intervention strategies, this workshop teaches participants how to prevent and prepare for psychological trauma, helps to reaffirm both the physical health of members of the school community and students’ perceptions that they are safe and secure, evaluates the degree of psychological trauma, responds to the psychological needs of members of the school community and examines the effectiveness of school crisis intervention and recovery efforts.

Bring Diabetes to Light - Suffolk

November is National Diabetes Awareness Month and JDRF will commemorate it by raising awareness of type 1 diabetes (T1D) when we join iconic buildings and sites throughout the world and light up in blue! The diabetes community will unite and draw attention to the impact it has on millions of Americans when we partner with the County Executives in a brief program and illumination of the buildings.

LGBTQ Health Webinar
Tuesday, November 27 @ 6:00 p.m.
As communities have become more diverse, the need to practice cultural competency and health literacy are essential to providing exceptional, patient-centered health care.

LGBTQ Health, an awareness-raising training, aims to reduce barriers to care by sharing information about cultural and linguistic competence, health disparities, and unconscious biases directly related to the LGBTQ population. This webinar will touch on topics surrounding implicit bias, LGBTQ 101, vocabulary, health disparities, and best practices for frontline staff.
 
WHO SHOULD ATTEND? Any persons working in human service organizations including, but not limited to hospitals, community-based organizations, and local health departments.

Youth Mental Health First Aid
Thursday, December 6 @ 8:00 a.m. - 2:30 p.m.
This program teaches school personnel, family members, social services and agency staff how to recognize and help a youth who may be developing the signs and a mental health disorders/crises in youth, or experiencing a mental health or substance abuse crisis. The 5-step Mental Health First Aid action plan is explored and applied to youth mental health disorders and crisis situations through the use of discussions, case examples and videos. This action plan includes approach skills, effective communication of concern and support, and connecting youth and families to professional and community resources.

11th Annual Suicide Prevention Coalition of Long Island Symposium
Tuesday, December 11 @ 8:00 a.m. - 3:30 p.m.
Dr. Christine Moutier, Chief Medical Officer of the American Foundation for Suicide Prevention, will help us make sense of the current body of data regarding risk of suicide among our youth and young adults, the rise in the rate of teen and young adult death due to suicide, and specifically, how we can use what is known about the impact of social media in our work to identify, address and reduce risk.  What are the conversations that we should be having with youth and young adults and how do we frame those conversations?  In addition, Dr. Moutier will discuss cyberbullying and risk of suicide. 5 continuing education hours available for LMSW/LCSW/LMHC/LCAT/LMFT.

Learning 2 Breathe Intensive Training
Monday, February 4 & Tuesday, February 5 @ 8:30 a.m. - 3:00 p.m.
This two-day workshop is designed to help you facilitate the growth of mindfulness in adolescents using the Learning to BREATHE (L2B) program. Demonstration, small-group practice and group feedback are done for each of the six themes of the program. Discussion about the role of the teacher and the pedagogical elements of mindfulness-based programs is part of the workshop. We will be engaged in a mix of demonstration, practice teaching and group discussion throughout both days. The workshop is intended to prepare participants to actually teach the program, so it relies more on practice than on presentation of theoretical and research background information. A short overview of the curriculum will be shared on day 1. All materials for the workshop will be provided. This training can be used to support the implementation of the amended New York State Education Law in Mental Health Education Literacy for schools.
