More Than $5.2 Million Awarded To Improve Care for Older Adults


The John A. Hartford Foundation is pleased to announce five new grants totaling more than $5.2 million to implement and disseminate evidence-based approaches to better care for older adults:
Join: A virtual expedition exploring the 4Ms of age-friendly care—What Matters, Medication, Mobility, Mentation—and how you can test them in your own setting, led by IHI.
