Million Hearts® hypertension champions
Million Hearts® has recognized 18 health care practices and community health centers as 2018 Hypertension Control Champions. In their work, these Champions ere able to achieve blood pressure control for at least 80 percent of their adult patients with hypertension. Together, the 2018 Champions cared for more than 53,000 adults. Read more about it here.
 
Medication trauma: What it is and how to help
Medication complexity is a challenge to patients, physicians and pharmacists alike. People with complex health and social needs are often juggling medications with no single entity coordinating the overall medication regimen. Despite these challenges, there is typically limited attention paid to the issue. The Center for Health Care Strategies recently spoke with Jim Slater, PharmD, executive director of pharmacy at CareOregon, to discuss medication trauma, the innovative tools his team uses to identify patients at risk, and how other organizations can address this issue. Read more here.

Healthy People 2020 and Diabetes Advocacy Partnership highlights opportunities to eliminate health disparities in diabetes
Healthy People 2020 (HP2020) is highlighting its strategic partnership with the Diabetes Advocacy Alliance to achieve the HP2020 Diabetes objectives over the decade. The objectives relate to preventing diabetes among those at risk and improving glycemic control among people with diabetes. Currently, 30.3 million people in the United States have diabetes — this includes both diagnosed and undiagnosed cases. Read more here.

Spanish language diabetes and hypertension ECHO series
Migrant Clinicians Network is hosting an ECHO Diabetes and Hypertension Series. Take advantage of this dynamic opportunity to become a part of a group of health workers from across the country dedicated to diabetes and hypertension care in the communities they serve.

The series will be presented in Spanish and is targeted for health promoters, community outreach workers, and case managers who work directly with patients in the detection, treatment, self-management, and prevention of diabetes and hypertension. Beginning Thursday, January 17, 2019, ECHO participants will meet weekly on Thursdays at 11 a.m. MT. Apply early as space is limited to 25 participants.

VISION data updates

  • Thirty-four health measures from the Healthy Kids Colorado Survey (HKCS) have been updated in VISION with 2017 data.
  • It was Vape Free November last month in Colorado, so the prevalence of current e-vapor product use among high school students (2017 HKCS) has been highlighted on the Data by Demographics tab.  
  • The most recent data (2016 or 2017 BRFSS) are also now available on the Multiple Risk Factors among Adults tab.
 

Introducing new physical activity guidelines for Americans
The U.S. Department of Health and Human Services has released the Physical Activity Guidelines for Americans, 2nd edition, which outlines the amounts and types of physical activity needed to maintain or improve overall health and reduce the risk of chronic disease. The document also highlights new benefits of physical activity and tested strategies that can be used to get all Americans more active.

Online training: Pharmacist Collaborative Practice Agreements
Pharmacists are well positioned to assist health care teams in the treatment and management of chronic disease due to their expert knowledge, training in medication management, and high accessibility to the general public. A collaborative practice agreement (CPA) creates a formal practice relationship between a pharmacist and a collaborating prescriber.

This training is intended for pharmacists, who are eligible to receive free continuing education credits for completing the training. It is also intended for prescribers, decisionmakers, and public health practitioners who are interested in expanding team-based care and developing CPAs.


Strategies for behavioral health integration: defining roles and responsibilties ECHO series
Integrating behavioral health into primary care creates new challenges to navigate. This series will assist behavioral health providers in developing strategies, implementing and operationalizing tools, and discussion ways to be a leader. This series runs for five weeks and begins on Feb. 6, 2019, from 12-1 p.m. MT. Learn more and register here.

The Community Clinical Linkages Toolkit

The Million Hearts® Collaboration, American Heart Association and other stakeholders have unveiled a new tool to promote community-clinical linkages: The Community Clinical-Linkages Toolkit. The Toolkit organizes an array of resources in a handy grid format to help public health practitioners implement the seven step-by-step strategies that are highlighted in the Community-Clinical Linkages Practitioner’s Guide under the LINKAGE framework.

American Diabetes Association provider videos

The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length.

Resource toolkit

As part of the patient’s health care team, pharmacists can improve disease outcomes by improving medication adherence and providing patient planning, education and counseling. The Community Preventive Services Task Force found strong evidence that team-based care can improve blood pressure control when a pharmacist is included on the team. A new toolkit from The University of Colorado provides sample agreements and resources helpful in integrating clinical pharmacists into the care team. Click for more information.

Updates in Pharmacist Cardiovascular Care online study modules

The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, with support from the Colorado Department of Public Health and Environment, created a series of online educational modules that highlight the role of pharmacists in the prevention and treatment of cardiovascular disease. To view the modules:
  1. Navigate to ipharmCE.UCDenver.edu (or http://ipharmce.ucdenver.edu/student/exam). You will need to create a profile if you do not have one. To create a profile, enter your email under “New Account Sign-up,” and follow the instructions on the screen.
  2. Once logged in, click “View All Programs” under Home Study Activities on the left side of your screen.
  3. Under the group Updates in Pharmacist Cardiovascular Care, click on the individual presentation titles.
