Medical Physics Tools, Treatment Planning and Dosimetry special feature
East London Trust enhances QA workflow with excellence in quality assurance and treatment plan system verification

Barking, Havering and Redbridge University Hospitals NHS Trust (BHRUT) has updated its Quality Assurance (QA) portfolio to enhance QA workflow and treatment plan system verification. The range of systems in use with others soon to be deployed include COMPASS and Dolphin Patient QA from IBA, ImSimQA™ from Oncology Systems Limited and Modus Medical’s QUASAR™ Penta-Guide Phantom.

The radiotherapy department is using the myQA global quality assurance system from IBA to connect all its QA applications, people and knowledge via a single cloud-based platform. This also provides access to IBA’s Compass, for 2-in-1 patient dose QA in 3D patient anatomy and the Stealth Chamber Reference Detector, a unique perturbation-free ‘beam invisible’ reference signal chamber for relative dosimetry. The Dolphin® Online Ready technology has also been selected to ensure efficient pre-treatment patient QA process. Read more on the OSL website.

The PTW T-REF chamber – the new reference detector for small field dosimetry?

The 34091 T-REF chamber is a large-area plane-parallel transmission reference chamber proven to offer an excellent reference signal for the measurement of small fields. It provides a solution to the problem of where to put a reference detector in small fields: it is big enough to accommodate small fields of up to 5cm × 5cm to fully transmit through the chamber without being disturbed at the field edge.

Using a dedicated holder attached to the water tank1 wall, it is positioned at a minimum distance to the water surface of 20cm with no perturbations to the beam. The very good signal-to-noise-ratio makes it an excellent option for the use as a reference detector in small field dosimetry.

Universal chamber holder will attach to any tank wall.

·         low total area density of 206 mg/cm², water equivalent thickness of 2.06mm

·         perturbation free transmission chamber

·         exceeds signal-to-noise-ratio of most vented chambers on the market

·         easy to mount to water phantom without contact with linac head

Optimized DCAT promises much faster SBRT delivery versus VMAT for many patients

Monaco 5.1 for Dynamic Conformal Arc Therapy (DCAT) improves plans through Segment Shape Optimization and Variable Dose Rate

Optimized DCAT is a unique approach to DCAT treatment planning available only in Monaco.

Steep reduction in monitors units significantly reduces treatment time
Patients with simple lesions not close to OARs are ideal candidates.

At UT Health San Antonio, as many as 40 to 60 percent of patients who would normally receive SBRT with VMAT delivery could be candidates for SBRT plans delivered with optimized DCAT, predicts UT Health physicist Sotiri Stathakis, PhD. The benefit for these patients – those who can’t tolerate a long therapy session and/or who present simpler cases that do not require the extreme modulation of VMAT – is a DCAT beam-on time as much as 2.5 times less than that of a VMAT fraction yet with comparable plan quality.

Dosimetric comparison of single-beam multi-arc and 2-beam multi-arc VMAT optimization in the Monaco treatment planning system

The purpose of this study was to evaluate the dosimetric and practical effects of the Monaco treatment planning system “max arcs-per-beam” optimization parameter in pelvic radiotherapy treatments. Read the full paper here.
