Hypertension Control Challenge
Call for Applications: 2018 Million Hearts® Hypertension Control Challenge!
Many practices and systems work with their patients to achieve blood pressure control. The 2018 Million Hearts® Hypertension Control Challenge will identify clinicians, clinical practices, and health systems that have exceptional rates of hypertension control.
Health professionals, practices, and health systems that have achieved hypertension control rates of at least 80% are eligible to enter the 2018 Million Hearts® Hypertension Control Challenge. The submission deadline is April 6, 2018.

American Diabetes Association Alert Day® is March 27
What’s Your Type 2 Diabetes Risk?
One in three American adults is at risk for developing type 2 diabetes, a serious disease that can lead to complications like kidney failure, heart disease, stroke, blindness, and amputations. But type 2 diabetes doesn’t have to be permanent—it can be prevented or delayed with healthy lifestyle modifications. The first step is learning your risk.
Take a simple and anonymous one-minute test to find out if you are at risk for developing type 2 diabetes. You’ll answer questions such as, “Do you have a family history of diabetes?” and “Are you physically active?” to learn your diabetes risk in 60 seconds. It’s that simple.
Once you’ve taken the test, share it with friends and family—with 84 million Americans at risk for developing type 2 diabetes, someone you love could be at risk.

Diabetes Self-Management Utilization Sharing Calls
Every third Wednesday of the month
8:30 - 9:00 am OR 12:00 - 12:30 pm
The Colorado Department of Public Health Environment and Telligen co-host these sharing calls to discuss barriers to DSME utilization, share successes, and identify tools that may be helpful in increasing DSME utilization throughout the state. The intended audience is ADA recognized or AADE accredited program staff, however, all are welcome to participate. DTTAC™ Virtual Lifestyle Coach Training
DTTAC is now offering Lifestyle Coach training virtually. DTTAC™ Virtual Lifestyle Coach Training is a four-week, interactive and innovative virtual training that meets CDC training standards and provides Lifestyle Coaches with the skills, knowledge and experience needed to successfully facilitate the National DPP lifestyle change program.
The virtual training provides you with the same high quality experience and in-depth learning as the in-person training but is done more conveniently over smaller pockets of time, allowing for more reflection and application, while saving organizations more money by removing added travel costs for participants.
The cost of the training is $649 per person plus participants receive free access to a recorded Lifestyle Coach Refresher webinar as a part of ongoing continuing education recommended by CDC. Continuing education credits have been applied for and will be available for RD, CDE, CHES, RN and RPh/PharmD for an additional $25.

New videos help people with diabetes better manage their health
The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) released new videos that offer support to people with diabetes, and steps they can take to manage their disease.

American Diabetes Association Provider Videos
The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length.

Resource Toolkit
As part of the patient’s health care team, pharmacists can improve disease outcomes by improving medication adherence and providing patient planning, education and counseling. The Community Preventive Services Task Force found strong evidence that team-based care can improve blood pressure control when a pharmacist is included on the team. A new toolkit from The University of Colorado provides sample agreements and resources helpful in integrating clinical pharmacists into the care team. Click for more information.

Updates in Pharmacist Cardiovascular Care Online Study Modules
The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences with support from the Colorado Department of Public Health and Environment created a series of online educational modules that highlight the role of pharmacists in the prevention and treatment of cardiovascular disease. To view the modules:
- Navigate to ipharmCE.UCDenver.edu (or http://ipharmce.ucdenver.edu/student/exam) You will need to create a profile if you do not have one. To create a profile, enter your email under “New Account Sign-up,” and follow the instructions on the screen.
- Once logged in, click “View All Programs” under Home Study Activities on the left side of your screen
