Stonegate CHC has been providing primary health care, health promotion and a wide variety of community programs to the Stonegate - Parklawn Community since it was created by community members in 1992.

2018 Summer Newsletter

Moving Mid-August to our Beautiful New Home

Stonegate CHC is very excited to see our new home taking shape.  It will be ready very soon. We will move into the full second floor of the building in mid-August.    As soon as we have our moving date confirmed, we will communicate it to the community in a variety of ways.  There will be some interruption in service to allow our move to occur.  Most programs will be cancelled for a five day period over one weekend.    We will have some urgent care appointments available during this period and as in after-hour periods; patients can have a telephone consultation with one of our providers if they have a concern.
 
The new building has much greater space for our health services and health promotion programming.  We will have five different, fully accessible spaces that can host simultaneous programs and we will be looking at expanding our partnerships to invite more programming into this beautiful, light filled space. 
 
One large room we are dedicating to physical activity and we are able to fit it out and equip it with a grant from the Ontario Trillium Foundation.  We will engage the community to learn what fitness, dance and other programs would encourage them to participate in more physical activity.  There will also be a teaching kitchen where community members can gather around an island to learn about ideas to get more nutrition with their food dollars.  Another room is dedicated to children’s programming for babies, toddlers and after school programming.  A large multipurpose room will host a wide range of programs – youth programs, prenatal classes, women’s programs, seniors programs – all our programs aim to increase community engagement, reduce isolation and promote healthy nutrition and active living.  A meeting room will be available for community members to gather to meet, but also to learn skills such as English as a second language, sewing and quilting.
 
We do have a fundraising campaign to help us fully equip all of these program spaces on our website www.stonegatechc.org
 
We plan to host a community meeting in our new building at 1:00 on September 22, 2018 just after our Annual General meeting at 11:00 a.m.  We will give tours of our new centre and engage the community about community needs they want to have addressed and ideas for programming at our Community Health Centre as we develop our Strategic Plan for the coming years.


Strategic Planning

Stonegate CHC’s Board of Directors has begun the process toward establishing the Strategic Directions for the coming years.  We hope to complete this process by December 2018.

A good strategic plan addresses three basic questions:

  • What do we want to achieve three to five years from now?
  • What is the environment? Specifically, what factors in the external and internal environment might support us? What do we need to avoid or overcome?
  • How do we get there? What are our overall directions or goals and how do these translate into clear measurable objectives so that we will be able to assess our progress along the way?
The Board began by reviewing and revising our Mission, Vision and Guiding Principles and plan to take these to the Members at the Annual General Meeting on September 22, 2018 for their consideration.

They also reflected on Stonegate CHC’s strengths, weaknesses, opportunities and threats at a session with the Board of Directors and in addition with a session with the Staff of Stonegate CHC.

We will be gathering and mapping information about the community we serve through a review of community statistical data, data about the clients we now serve and comments from clients on surveys they completed.

We want to invite interested community members to a meeting in the fall on September 22, 2018 at 1:00 in our new building at 10 Neighbourhood Lane.  We will give community members tours of the new Centre and hope to engage you to help us understand the needs of this community and help us shape our future directions.

The result will be a Strategic Plan that the Board of Directors will commit to for the next 3 to 5 years which will help guide and shape our future.

Community Health Champions

We are excited to participate in a new evidence based initiative that has proven to be very successful in the United Kingdom.  

Social prescribing is a new spin on an old idea - that people are healthier when they are connected with social resources and supports in their communities.  Using the Community Health Champion model we will reach out to individuals in our community to help us engage, support and inspire members of our community to lead more healthy lives.  Community Health Champions are individuals that want to share their skills and life experience to give back to the community by carrying out various roles including health education, outreach, social support and group activities. 

Do you want to give back to your community? Do you want to be a peer mentor or lead a group activity?  If you responded “yes” then stay in touch with us, this fall we will be looking for Community Health Champions and you may be one of them!

Stonegate Farmers' Market 2018 Season  

The Stonegate Farmers' Market will open its 13th season on Tuesday, June 19th! The Market will run Tuesdays from 4:00 - 7:00 pm (rain or shine) from June until October, featuring: local produce, meats, cheeses, honey, jams & jellies, sauces, baked goods, breads, and more!

Located in the parking lot of Christ Church St. James Anglican Church at 194 Park Lawn Road (just north of the Queensway), the market showcases fresh produce direct from the farmers (conventional and organic) with local entertainment, special events, a market BBQ, and children's activities throughout the season.

During the month of July at the Market, customers will have the opportunity to purchase market bucks. For every $20 purchased, you will receive an additional $5 in market bucks free to spend (of course at the market). A maximum of $40 per market can be purchased per customer ($50 in market bucks).

Arts Etobicoke:  Bell Manor Park Party and More! 

Arts Etobicoke and Stonegate CHC invite you back for the second annual Bell Manor Park Party on Friday, September 14th, 2018, 3:00 - 6:00 pm! There will be delicious, local food, fun activities, and amazing artists.

Leading up to the Park Party will be programming for Stonegate residents of all ages. This includes outdoor theatre workshops facilitated by the incredible Young People’s Theatre in conjunction with the Stonegate Youth Summer Camps as well as a public art installation presented by arts organization, STEPS Initiative, and will be open for public participation.
 
This art installation will utilize the community garden fence and YOU can join in on the fun! Art workshops will take place in the park over July & August and are opportunities for community members to connect and get creative!
 
Help design and install the art piece with your neighbours on the following dates. Spaces are limited, so act fast!

Design Day:  July 18th  @ 1:00 - 3:00 pm – Picnic Tables in Bell Manor Park 
Come get creative with your neighbours and learn how to sketch your design ideas for the fence installation. Artist-facilitator Bereket Kezwer will use the designs as inspiration for the community art project for all to see!   To participate in the public art workshops hosted by STEPS, registration is required. Please contact Stonegate to secure your spot at: 416-231-7070 ext. 307.

First Installation Date:  August 16th @ 10:00 am - 12:00 pm – Picnic Tables in Bell Manor Park
For this collaborative group activity, artist Bereket Kezwer will present the new fence art design and community members have the opportunity to install the design themselves!
