Coming soon: Million Hearts® Cardiac Rehabilitation Change Package
Coming in June, CDC, in collaboration with the American Association of Cardiovascular and Pulmonary Rehabilitation, will be releasing the Million Hearts® Cardiac Rehabilitation Change Package. This quality improvement guide is a menu of evidence-based strategies and related tools and resources for improving cardiac rehabilitation utilization. It is intended as a tool for anyone interested in improving rates of referral, enrollment, participation, and adherence.

Changing how blood pressure is measured will save lives
Traditional methods of testing for high-blood pressure are no longer adequate and risk missing vital health signs, which can lead to premature death, a study co-led by UCL has found.

National Diabetes Prevention Program (National DPP) maps in United States counties is now on the Division of Diabetes Translation (DDT) website. The maps show county level availability of in-person National DPP programs, overlaid with diabetes incidence and socio-economic status.

Diabetes Self-Management Utilization Sharing Calls
Every third Wednesday of the month
8:30 - 9:00 am OR 12:00 - 12:30 pm
The Colorado Department of Public Health Environment and Telligen co-host these sharing calls to discuss barriers to DSME utilization, share successes, and identify tools that may be helpful in increasing DSME utilization throughout the state. The intended audience is ADA recognized or AADE accredited program staff, however, all are welcome to participate. Coverage for the National Diabetes Prevention Program: Employers and Commercial Health Insurance Plans

This webinar will focus on specific steps that employers and commercial health plans can take to effectively establish coverage for the National DPP lifestyle change program. It will include a brief overview of the CDC-led National DPP and its impact nationwide. This will be followed by a panel discussion with representatives from the National Alliance of Healthcare Purchaser Coalitions (National Alliance), the Houston Business Coalition on Health (HBCH), Dow Chemical, and Rice University, all of whom will share their experiences and perspectives on covering the National DPP lifestyle change program.
Following the panel discussion, NACDD and Leavitt Partners will share information and expertise gained from research into commercial and employer coverage models, and demonstrate how to access helpful resources on the National DPP Coverage Toolkit.

DTTAC™ Virtual Lifestyle Coach Training
DTTAC is now offering Lifestyle Coach training virtually. DTTAC™ Virtual Lifestyle Coach Training is a four-week, interactive and innovative virtual training that meets CDC training standards and provides Lifestyle Coaches with the skills, knowledge and experience needed to successfully facilitate the National DPP lifestyle change program.
The virtual training provides you with the same high quality experience and in-depth learning as the in-person training but is done more conveniently over smaller pockets of time, allowing for more reflection and application, while saving organizations more money by removing added travel costs for participants.
The cost of the training is $649 per person plus participants receive free access to a recorded Lifestyle Coach Refresher webinar as a part of ongoing continuing education recommended by CDC. Continuing education credits have been applied for and will be available for RD, CDE, CHES, RN and RPh/PharmD for an additional $25.

The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) released new videos that offer support to people with diabetes, and steps they can take to manage their disease.

American Diabetes Association Provider Videos
The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length.

Resource Toolkit
As part of the patient’s health care team, pharmacists can improve disease outcomes by improving medication adherence and providing patient planning, education and counseling. The Community Preventive Services Task Force found strong evidence that team-based care can improve blood pressure control when a pharmacist is included on the team. A new toolkit from The University of Colorado provides sample agreements and resources helpful in integrating clinical pharmacists into the care team. Click for more information.

Updates in Pharmacist Cardiovascular Care Online Study Modules
The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences with support from the Colorado Department of Public Health and Environment created a series of online educational modules that highlight the role of pharmacists in the prevention and treatment of cardiovascular disease. To view the modules:
- Navigate to ipharmCE.UCDenver.edu (or http://ipharmce.ucdenver.edu/student/exam) You will need to create a profile if you do not have one. To create a profile, enter your email under “New Account Sign-up,” and follow the instructions on the screen.
