Patient Success Story

"I had little chance of survival. Now I'm cancer free and able to do everything I used to do before."
MyClinic patient Antulio is a cancer survivor. Click here to hear his inspirational story.
Meet Antulio Quinonez Mendoza, a 32-year old colon cancer survivor. This young man was referred to MyClinic by El Sol- Jupiter’s Neighborhood Resource Center, in order to investigate his feeling weak and abdominal pains. While initial bloodwork revealed he was suffering from severe anemia, later, a colonoscopy and endoscopy revealed shocking news - he was diagnosed with stage 3 colon cancer! Thanks to the dedicated assistance of MyClinic’s Patient Care Coordinator and our partners at Jupiter Medical Center, in particular the doctors of Ella Milbank Foshay Cancer Center and Jupiter Medical Specialists, Mr. Mendoza received all necessary services - including urgent surgery, free of charge. He is alive today thanks to these interventions. Antulio allowed us to share his story with the community to stress the importance of early detection and give hope to others who do not have health insurance or the funds to pay for costly doctor's appointments.

At a Glance

In addition to Primary Medical Care, MyClinic is proud to offer preventive treatment, comprehensive chronic disease management programs, health education, behavioral health services, referrals for lab and imaging, medication counseling and assistance. In 2018, total patient encounters (as of July 31st) were 4800, a 47% increase over the same time period last year. Demand for MyClinic services continues to increase as an average of 34 new patients register and receive care every month. We could not accomplish what we do in our community without the countless hours and support we receive from our generous volunteers, partners and donors. We look forward to continuing our momentum through 2018 and beyond.  This year, so far, our dedicated volunteers have provided over 4,200 hours of service. Jupiter Medical Center continues to provide lab and imaging services free of charge to MyClinic patients as well as administrative support.  During the first half of 2018, In-Kind services and donated supplies from all sources is valued at approximately $500,000.

We are happy to announce the successful start and implementation of our Diabetes Triple Aim Initiative developed in partnership with the Institute for Healthcare Improvement and funded by Healthier Jupiter. The program is aimed at simultaneously improving the health of our population with type 2 diabetes, enhance the experience and outcomes of the patient and reduce the cost of care for the benefit of the community. The Triple Aim Initiative is designed as a 1 year-long program involving currently 12 patients with type 2 diabetes. Under constant monitoring by the Care Coordinator, Karen Herrera, the participants receive regular motivational phone calls and are advised with regard to their medication, nutrition and behavioral health. The group also meets twice per month in a support group setting which encourages participants to freely share and discuss their progress, failures and receive professional counseling on topics related to diabetes education and behavioral health such as stress management and making healthy lifestyle changes. The positive improvements and transformations of these patients so far would not have been possible without the help of a dedicated team of staff, providers, volunteers and partners such as Jupiter Medical Center who have provided the education component through their certified diabetes educator.

Alvaro is one of the patients taking part in MyClinic’s Diabetes Initiative. Through dedication, daily exercise and healthy eating, Alvaro has lost 35 lbs in just 3 months and his glucose levels are now spectacular!

"I am fortunate with the opportunity to providing counseling and behavioral health services to MyClinic patients, as part of a Quantum grant since November 2016. The labor of compassion and care that I have witnessed with services offered by dedicated staff and volunteers makes this one of the most pleasurable jobs I have had in my career. As part of a multidisciplinary team, we provide medical, nutritional, behavioral, case management and psychoeducational group support to 12 MyClinic patients for a period of 1 year. We are in our fourth month of the mini grant. The insight that many of the patients are developing as they share their personal challenges
and triumphs is impressive. The support group meets for 90 minutes and we start by introductions and how long have the participants been diagnosed with type 2 diabetes. The atmosphere is supportive and patients have been open and candid about their individual experiences and what has helped them understand their condition. The educational component of the group has included medical information about diabetes, stress management, exercise and nutritional information as well as addressing all participants and providing incentives for attending the monthly groups."

Partnering with FAU To Launch Scribe Program

MyClinic has partnered with FAU's Harriet L. Wilkes Honors College of Jupiter to launch a Scribe Program in fall 2018. Starting September, a group of 4-5 FAU undergraduate pre-med students will begin a semester-long program in which they will learn about, shadow, and become scribes. Scribes are responsible for recording all the information that the healthcare professionals observe and assess. This will allow them to gain first-hand practical experience by interacting with Myclinic patients, doctors and nurses. Selected applicants will get the chance to perform this critical job of helping keep medical records of patients as accurate as possible, enabling a continuum of care at the highest quality standards.

We expect this to be the beginning of a self-sustaining program in which experienced scribes are responsible for training new scribes. This will provide an invaluable opportunity to undergraduate students interested in pursuing a career in medicine, helping to push the future of medicine farther along their path. MyClinic has even contacted a clinic in Virginia having a similar initiative to gain insights into existing best practices.

Interested applicants should bring in a serious interest in pursuing a medical profession, time commitment (preferably able to volunteer up to senior year), willingness to learn and be coached, capacity of processing information quickly and in a comprehensive manner.

Volunteer Spotlight: Jason Baluja

What motivated you as an undergraduate student to volunteer with us?
The wonderfully altruistic work being done at MyClinic is one of the reasons I was initially interested in volunteering, but it was not my primary reason. Admittedly, it was to bolster my medical school application by racking up shadowing hours with the possibility of becoming a scribe.
You are fluent in both English and Spanish. Did this help you in any way?
Being bilingual allowed me to also become an interpreter for patients and providers at the clinic. It was in this role that I discovered the true magic and beauty of not only MyClinic, but of the medical profession in general.

Why do you consider a scribe's role of such great importance?
Scribes are basically note takers for the providers. One of the main hurdles to enrolling the expertise of providers is that many do not wish to and/or do not have the time to learn how to operate the computer programs of the clinic. Therefore, it is crucial to have multiple trained scribes to eliminate this hurdle. After expressing my scribing interest, I was even given the opportunity of creating a scribing program for MyClinic.

What is the most important thing you learned since volunteering here?
When I became responsible for interpreting provider’s thoughts, instructions, diagnosis, and explanations directly to the patient from English to Spanish, my perspective shifted.
I got something of even more value from the clinic. That something is passion.
It was no longer “I think this is what I would like to do,” it became “this is what I want to do.”
MyClinic did not only grant me an unparalleled opportunity to create something special of importance, it allowed me to discover what it truly means and feels like to be a part of a team that provides care for patients.

CIHP Auction Fundraiser

The CIHP (Central Institute for Human Performance) and the Medical Group of South Florida organized on February 23, 2018 a highly successful fundraising event on behalf of MyClinic. Under the motto "A Night Out with the Professionals", some of our region's professional baseball and golf players, healthcare professionals as well as some local businesses offered their services, products and gift vouchers to be auctioned during a silent auction. Even a VIP concierge medical care service valid for 1 year was offered by MyClinic's Vice Chair Dr. Ron Surowitz. We are hereby expressing our deep gratitude to the organizers in particular to Dr. Clayton Skaggs and his team at CIHP and to all participants for helping us raise over $18,000.

Upcoming Partnership w. Gregory School of Pharmacy of PBAU

MyClinic will partner with the Gregory School of Pharmacy of Palm Beach Atlantic University later this year in order to help bring in faculty members and students for monitoring the medication of patients with type 2 diabetes involved in the current Triple Aim Diabetes Initiative above mentioned.

`The New Tax Law and Charitable Giving

As we move closer to the end of the year, more people will start to think about how their charitable giving will be impacted by the Tax Cuts and Jobs Act. The new tax law nearly doubles the standard deduction in 2018 to $12,000 for singles and $24,000 for joint filers younger than age 65, while capping or eliminating other deductions. That means it will no longer make sense for many taxpayers to itemize. The Tax Policy Center estimates the number of households itemizing deductions for charitable giving will fall from about 37 million to about 16 million.

If you still want to support organizations like ours while also deriving as much benefit as possible,Kiplinger has a number of  suggestions, several of which are highlighted here:

Consider donating shares of appreciated stock or mutual funds. You can deduct the current value of the investment as a charitable contribution if you itemize, and you'll avoid paying capital gains taxes on the profits. Plus, the charity receives the full value of your investment.

If you're 70½ or older, you can transfer up to $100,000 from a traditional IRA tax-free to charity each year. It will count as your required minimum distribution (RMD) without being added to your adjusted gross income. Your charitable gift won't be taxed, as it would be if you were to take a distribution and then donate the cash to charity. Plus, keeping your RMD out of your adjusted gross income could help keep your income below the threshold for being subject to the high-income surcharge for Medicare parts B and D, as well as hold down the percentage of your Social Security benefits that are subject to taxes.

You can also name a charity as the beneficiary of your IRA. That way, neither your heirs nor your estate will pay income taxes on the assets, and the charity will receive the full value.

This material is intended to be informative, not tax advice. These are only some of the options available. You should consult a tax adviser for specific information.
