In order to help us prioritse from the high demand, we would ask you to complete all sections of the form below as fully as you can. By providing this information upfront you will give us a clearer idea of your circumstances and save our volunteers time. This enables us to help more people.

Please be patient while we respond, we aim to treat each person as an individual and take the time to ensure that each referral gets the time and attention they need. This will of course apply to you when the time comes for yours to be reviewed.

Below you will find options for marketing and photograph consent. The Hope Project would be very grateful if you would allow a photo to be taken of you with your new item. This will help us to produce newsletters and other promotional material showing the work of the Hope Project to try and raise awareness and much needed funding. By consenting you agree that your name and image may be used for the promotion of the Hope Project in the public media, newspaper, newsletters and online. The Hope Project may edit the material you provide and may also decide not to use it.
