The Monitoring and Support tool was created to guide Care Manager conversations with individuals and families in the community, and now, also residents in certified programs.
This common guidance was created by CCOs to facilitate extended stays for people who live in certified settings, but are on an extended home visit to inform our members and ensure they receive the supports they need during this time.
