My Health Plan - Manitoba Institute for Patient Safety

Do you dread the question, “what did you do last time?” when you arrive for help with a bleeding episode? To be fair, many of us don’t have the same type of bleed close together and we forget exactly what was done last time. Using a log to help document notes about a bleeding episode; what worked well and what didn’t, can be a huge help!  
 
My Health Plan is a set of tools developed by the Manitoba Institute for Patient Safety that can help you manage your bleeding disorder and other regular health issues. A health plan is much more than just patient records; it combines data, knowledge, and experience, which helps patients become active participants in their own care, and that can transform a patient-provider relationship!

Product Delivery Service 
 

If you need to have your medication delivered from CBS, please call CBS Blood Product Management at 204.789.1034 to check that they have your order.  Once that is done call the office at 204-775-8625* to arrange a courier. Please make sure that you are going to be home for 4 hours to accept the delivery. This service is for times when you are unable to collect the treatments yourself and is not meant to be used on an ongoing basis.

Delivery service is available Monday to Thursday. It is best to arrange delivery early in the morning to get same day service.

Self-Infusion Assistance and Training

Summer camp is when many young people try doing their treatment for the first time. We spoke to the Manitoba Bleeding Disorder Program nurses, Tracy and Crystalynne, and they don’t want you to miss an important window if you or your child is ready to learn self-infusion. You can make an appointment to have a training session with them. At this time, one patient and one accompanying person can come to an appointment.
