Greetings to the LaQshya-Manyata community!
Maharashtra’s State Public Health Department and Federation of Obstetric & Gynaecological Societies of India (FOGSI), have achieved an important milestone in their pursuit to benchmark quality of maternal care in Maharashtra by launching LaQshya-Manyata, a certification programme.
The initiative, launched in September, aims to establish a sustainable mechanism for quality improvement among private maternity care facilities, particularly those who cater to women in low and middle-income areas. In doing so, the program trains the staff of registered facilities on essential patient care, safety and facility improvement components based on a set of 26 standards of Quality of Care. These standards have been carefully identified by renowned private sector OBGYN specialists, covering components that are vital for delivering quality maternal care.
This is the first newsletter in the LaQshya-Manyata series. Through this bulletin, we aim to share with you the program progress, key highlights and recent achievements.

Developing countries have, for long, carried a high burden of global maternal deaths. The added tragedy is that these deaths are mostly preventable. In India, maternity-related complications and inadequate services to identify and manage them are leading causes of maternal mortality. The answer to avert these deaths is clear - make quality maternity care available, affordable and accessible for all women.

With that as the governing principle, Maharashtra’s State Public Health Department has been committed to strengthen the healthcare ecosystem, such that all women receive safe and respectful maternity care.We have made significant progress through our public health initiatives towards improving quality and coverage of maternal healthcare services in the state. With this success, we are now poised to further expand our efforts by taking strategic steps towards effectively integrating the private sector into the maternal care ecosystem. This move, we believe, is going to bring a step-change in the state’s service delivery for maternal care.
Maharashtra has a thriving private healthcare sector, for 45% of the institutional deliveries in the state. With limited regulation for private maternity healthcare providers, it is critical to create mechanisms for them to self-regulate for quality improvement and assurance. Taking cognizance of this and recognizing the work being done by Federation of Obstetric & Gynaecological Societies of India (FOGSI) through the Manyata initiative, the State Public Health Department in Maharashtra identified them as a crucial partner in further strengthening the quality care movement in Maharashtra.
After multiple meetings, discussions and deliberations, with unified goal to transform the maternal healthcare delivery system in Maharashtra, State Public Health Department and FOGSI launched LaQshya-Manyata on September 8th, 2019, in Pune. The launch was officiated in the presence of Hon’ble Minister Shri Eknath Shinde, Former Minister of Public Health and Family Welfare, Maharashtra.
Based on a public-private partnership model, the joint initiative focuses on quality improvement and certification of private maternity facilities in the state. In doing so, the LaQshya-Manyata team has been working tirelessly to ensure facilities, across all districts and Municipal Corporations of Maharashtra, are trained to deliver high-quality and respectful care to women.
I would like to thank the department, FOGSI and all partners for their relentless commitment and support towards the program. I am sure through our hard work and perseverance we will make Maharashtra a safe state for childbirth.

The program began to take shape in late 2018 & April 2019 with a series of discussions between Maharashtra's State Public Health Department and FOGSI. After these initial meetings, the Public Health department and FOGSI leadership decided to partner together and improve the quality care of private maternity hospitals in the state. Next steps included signing an Memorandum of Understanding (MoU), undertaking a joint workshop to decide scope of work, defining set of procedures, creating assessment structure, initiating the formation of operational and technical committee as well as finalizing the joint certification mechanism.
Before the launch, the committees consisting of renowned private practitioners & state government officials developed a compendium & training material on quality care in English and Marathi.
The Public Health Department also ensured that training facilities were made available for capacity building sessions. They also organized multiple orientation sessions for government assessors to orient them into the new program .
To ensure easy access to program information, the team unveiled a new LaQshya-Manyata website. Providers from Maharashtra can register themselves on the website and avail the benefits of the program.

SENSITIZATION & TRAINING SESSIONS
|

With the support of our on-ground team, sensitization sessions for the LaQshya-Manyata program have been organized for Obstetrics and Gynaecology (OBGYN) societies, across Maharashtra. Additionally, Manyata state champions have also been hosting sessions to enhance awareness among the owners of private maternity units around the program.

- LaQshya-Manyata CME was organized by Palghar and Chalisgaon OBGYN societies, on January 16 and 17, respectively. The sessions drew a lot of interest from doctors. They were keen to understand the new guidelines under the LaQshya-Manyata initiative endorsed by FOGSI and Government of Maharashtra.
- An orientation session on LaQshya-Manyata was held at Kolhapur OBGYN society on February 26, with 20 doctors in attendance.
- Dr. Milind Shah, a Manyata lead assessor, hosted a LaQshya-Manyata orientation session, on Feburary 26, at Karad OBGYN society. 29 doctors attended the session, many of whom later expressed interest in joining the program.
- In the month of April, Dr. Sanjay Gupte facilitated online sensitization sessions for Presidents/Secretaries of OBGYN societies in Akola, Wardha, Chandrapur, Dhule, Sagli, Hingoli and Pandharpur.

Orientation on LaQshya-Manyata

Multiple orientation sessions were conducted by Government of Maharashtra and FOGSI, in the last few months -
- January 4, 2020: Govt. of Maharashtra’s LaQshya Cell led a session to apprise district civil surgeons on the program’s 26 standards and the process of assessments to receive LaQshya-Manyata certification. The online session was also attended by President and Secretaries of local OBGYN societies of Maharashtra.
- January 9, 2020: A joint meeting was organised under the chairmanship of Civil Surgeon of Nanded, Presidents and Secretary of Nanded OBGYN society. Close to 42 members from the Nanded OBGYN society attended the session to understand the program.
- January 22, 2020: Government of Maharashtra & FOGSI conducted a joint meeting in Pune to orient assessors about the LaQshya-Manyata initiative. The session focused on sensitizing assessors on clinical & facility standards of the program. Close to 130 assessors from Public health department and 20 assessors from FOGSI attended the session.

Trainings under LaQshya-Manyata
With registrations pouring in for LaQshya-Manyata trainings have begun in full swing -
- Two training sessions, divided in two batches were organized in Bhandara and Kolhapur on January 14-15 and January 30-31 respectively. Over 90 participants in attendance, received hands-on training on the 26 Clinical and Facility Standards.
- At the Government training hall in Nanded, on February 25 & 26, over 22 nurses and staff members received training on the program’s 26 Clinical and Facility Standards, from the Civil Surgeon and Resident medical officer.
- Keeping the COVID-19 crisis and the subsequent lockdown in mind, virtual trainings were initiated, for the registered facilities, using Manyata ECHO platform. Our partner Jhpiego, has been supporting with the on-boarding of facilities, such that they can effectively use the digital platform.

Announcing ‘LaQshya-Manyata’, a joint initiative by Government of Maharashtra & FOGSI
The Hon’ble Minister for Health, Maharashtra, Shri. Eknath Shinde, Principal Secretary - Public health department, Government of Maharashtra, Dr. Pradeep Vyas, Director Health Services, Pune - Government of Maharashtra, Smt. Dr. Archana Patil & FOGSI President Dr. Nandita Palshetkar launched the program on September 8, 2019 in Pune. In attendance were prominent state government officials, the FOGSI leadership & MSD for Mothers.
LaQshya-Manyata is poised to step up the momentum for quality care in Maharashtra by helping to improve and standardize quality of care for private providers, particularly those who cater to women in low and middle-income areas. Providing equitable and inclusive healthcare, the program will also ensure consistent, safe and respectful care for mothers, during and after childbirth.

LaQshya-Manyata formal announcement by FOGSI
In January, FOGSI officially announced its partnership with Public Health Department, Government of Maharashtra, to improve quality of care in private maternity services in the state, through LaQshya-Manyata. The press meeting was hosted by FOGSI leadership, Manyata core team and MSD for Mothers. The news of the LaQshya-Manyata launch was covered extensively and made headlines across regional, national and online media.

LaQshya-Manyata at All India Congress of Obstetrics & Gynaecology (AICOG) Lucknow
LaQshya-Manyata initiative was introduced and presented at the 63rd All India Congress of Obstetrics & Gynaecology (AICOG), 2020 at Lucknow in January. At the event, FOGSI leaders, Government of Maharashtra team and program champions were felicitated with ‘LaQshya-Manyata Anchor’ and 'Excellence in Leadership' awards for their instrumental role in driving the LaQshya-Manyata initiative. A series of activities were organized during the course of five days to enhance the awareness of the program.

LaQshya-Manyata at Association of Maharashtra Obstetrics & Gynaecological Societies (AMOGS) Conference
In February 2020, LaQshya-Manyata was extensively showcased in the 3-day conference at Amravati. A branded LaQshya- Manyata stall was set-up at the venue to sensitize providers about the program. In addition, Dr. Hrishikesh Pai made a presentation explaining the model of the program. Our presence at this event translated in generating leads for program registrations. Moreover, post the event, many presidents/secretaries of local OBGYN societies invited us to make presentations in their upcoming Continuing Medical Education (CME) activities.

Special COVID-19 Sessions for LaQshya-Manyata providers
Throughout April, NPMU actively used the digital platform to sensitize providers about LaQshya-Manyata. In doing so, they conducted complementary technical sessions from the program modules, for members of the OBGYN societies in Akola, Chandrapur and Wardha. State champions, Dr. Sanjay Gupte and Dr. Rahul Wani, also conducted sessions on Infection Prevention Protocols/ PPH Management.

Summary: FOGSI published the revised COVID-19 recommendations for pregnancies with COVID-19 infection. They have also invited all its members to provide their suggestions and inputs on the document.
