We are seeking your input on our strategies and priorities as part of the process of completing a State Plan on Aging for 2020-2024.
You can take the short, five-minute survey by clicking on the button below. The survey is available now through June 26, in both English and Spanish and is open to everyone.
If you know someone who may not have internet access and who may be interested in taking the survey, they can call the department at 717-783-1550. The person should leave their contact information and indicate they want to participate in the ‘State Plan on Aging Community Survey.’ A department staff member will call them back to complete the five-minute survey over the phone.
