DSAMH has been working diligently to meet the needs of Behavioral Health clients during the COVID crisis.  COVID-19 has made the barriers to treatment even greater and the need to focus on social determinants of health.  Our Health Integration and Social Determinants (HISD) Bureau focuses every day on these issues by linking Delawareans who are engaged in mental health and addiction treatment with wraparound supports including:  peer services, housing, employment, transportation, legal assistance, as well as eligibility and enrollment, case management, and primary care integration.

HISD successfully opened Delaware’s first integrated recovery housing model in 2018. The integrated recovery housing model was designed to help ensure individuals in treatment receive full support during their recovery journeys. This model has resulted in positive outcomes for DSAMH clients and has garnered national attention.  HISD applied the model to protect over 330 vulnerable behavioral health clients from contracting and spreading COVID-19 in the State of Delaware during the height of the pandemic.

HISD partnered with two community providers, Aquila Delaware and RI International, to replicate the integrated recovery model in a hotel environment to allow for compliance with CDC guidelines related to social distancing and quarantining. This statewide, targeted outreach effort created an opportunity for unsheltered clients to connect to low barrier treatment services, while protecting their health and safety.  The hotel-based program operated for three months during which time the clients were assessed and provided with discharge plans that included ongoing engagement. Over 30 clients were trained as peer navigators and were offered jobs with benefits.  As lock-down restrictions lifted across the state, the majority of the clients in the program were able to transition to the next appropriate level of care. 

The entire HISD team continues to be proud to represent DSAMH in the community and is elated that members of our vulnerable population had the opportunity to safely engage in behavioral health services during a time of substantial uncertainty.

Client Show / No-Show 
 
Recently, a new enhancement was added to OpenBeds that allows providers a way to indicate whether or not a patient showed to their appointment.  This feature allows the providers to ‘close the loop’ on a referral and ensures a warm handoff for your client.  Since this feature went into place, together we have connected over 4,500 patients to care. Additionally, for those providers that have used the 'Close the Loop ' feature, 77% of all patients are making it to their appointments.

Providers will be able to 'close the loop' for a referral on the Referral Request Messaging page. In the status section, a provider will be able to check ‘yes or no’ to indicate that a patient showed for their appointment or did not show for their appointment.




Once a referral has been updated, providers will be able to see that a patient showed or did not show in the Status column on the Referral Request Status page in OpenBeds.

Multiple Referrals

Earlier this year, new functionality was added in OpenBeds that allows providers to submit a request for a patient’s referral to up to 3 facilities at the same time.  Providers will see the following checkbox option on the Service Availability screen.

After selecting the checkbox, providers will be able to select up to 3 facilities from those organizations listed on the Service Availability screen. Providers will see a Facilities Selected bar at the bottom of the screen that shows the number of facilities that were selected.

After clicking on the Submit Request button, the Referral Request screen will open. Providers will see those selected facilities under Contact Information. Providers will complete the referral. If a provider decides that a selected facility needs to be removed, the provider will be able to click on ‘Remove’.
Once the referral is submitted, each facility will receive a notification. When one of the facilities has accepted the referral, the other facilities will be able to see whether or not the referral was accepted. The referral will indicate that the patient has been accepted elsewhere in the Status column on the Referral Request Status page.
